Diagnostic Lab Data: Lab work on 3/2/01: LFT''s, chemistry, CBC, RPR, TSH, B12/folate, FTA-mildly low hgb of 11.6 gm/dL, automated eosinophil % of 4 (nl 0-3; ANC=230) and mild AST elevation of 45. On 3/13/01: CSF studies; MRI head/spine-showed "2 lesions in the CNS and 2 in the spine". On 5/19/01: MRI-showed multiple white matter signal abnormalities in a pattern compatible with multiple sclerosis and also suggestive of active demyelination. On 7/17/01: WBC-5400 cells/mm3, nl hbg, nl plts, increased eosinophil count of 1242 cells/mm, nl chem 7, ESR 15 mm/hr, LDH 567 (313-648), CRP of 0.15 (0.03-5.00nl), albumin 2.8, calcium of 8.1, total protein of 5.6 g/dL. Repeat CBC, after 3 days of IV Solu-Medrol hand no change in eosinophil count; CXR PA/Lateral was nml. Bone marrow biopsy

Write-up: Pt referred via consultation while she was on the Neurology Inpatient Service for evaluation of her "initiation of symptoms following vaccinations during basic training. She presents with concern for development of MS symptoms, post vax, including incidental finding of persistent eosinophilia. She had a nagging toothache for 2-3 months when she joined the US Army on 2/5/01 and it worsened and a tooth abscess was drained 3 days later. A small tooth chip was removed and stitches placed and she was placed on ibuprofen for pain. On 2/12/01, she received 5 shots and within 30 minutes, post vax, she developed a sensation of her right foot "falling asleep". This persisted and 2 days later, had increased to now involve her left leg but she was still able to do physical training (even passing a 1-mile run uphill). Over the next 2 weeks, the "numbness" sensation progressed to involve both hew lower extremities and symmetrically went up to her costal margin bilaterally. She was evaluated with the assessment of paresthesias and suspected neurological disease and neurology consult. She was admitted to the hospital on 3/12/01 but transferred to another hospital on 3/13/01 to 3/20/01 for "increasing symptoms". She was evaluated by neurology and tests. Dx at that time was encephalomyelitis secondary to vaccination. She was treated with high dose steroids. Prior to admission she also complained of headache, nausea and fatigue which almost completely resolved with steroid course. Her paresthesias also diminished although were now confined to her right leg and trunk. She was started on Elavil and Motrin for pain on 4/27/01 and Neurontin and Septra on 4/30/01. She was placed on convalescent leave although during a road march 5-6 weeks after entering basic she developed slurred speech and facial droop and was dx''d with Bell''s Palsy. She also had difficulty urinating. The dx of suspected MS was made and she again was treated with IV Solu-Medrol X 5 days and then a 2 week steroid taper. After review of her labs, she was again admitted to the hospital on 7/17/01. During her admission, Allergy/Immunology was consulted to address her vaccine issues and eosinophilia. Summary of symptoms since vaccines: paresthesias X 5 month; headaches X 2 months; fatigue X 5 months; pruritus X 5 months; light-headedness X 1 month; nausea X 3.5 months; intermittent emesis with headache; intermittent Bell''s Palsy 4 times. On 8/1/01, was also dx''d with spastic bladder secondary to MS and perennial rhinitis
