Write-up: Information has been received from a registered nurse concerning an 18 year old female with no allergies and with a urinary tract infection who on 12-JUN-2007 was vaccinated intramuscularly with the third 0.5 mL dose of Gardasil, (Lot number known by the"reporter). Concomitant therapy included /"birth control/" /"antibiotic/" (unspecified) and sulfamethoxazole/trimethoprim (BACTRIM).
