Diagnostic Lab Data: CBC, Lumbar puncture, blood and urine culture, drug screen, Ct scan brain were done. Cultures and PCR (CSF) negative. EEG NL.

Write-up: 5/6/05 HBV and Menactra were administered. 5/11/05 bronchitis, treated with Biaxin/Albuterol and Prednisone; 5/14/05 Diarrhea, vomiting; 5/15/05 generalized seizure, ER, Dx: viral meningitis. Pt 15 yo child was seen in our office on 5/6/05 for well visit."He was given HBV and Menactra. No calls received from parents regarding any side effects. On 5/11/05 child was seen for cough, wheezing. He was diagnosed bronchitis/RAD, started on Biaxin XC, Albuterol Inhaler and Prednisone for 5 days. As per mom, he dev
