Write-up: Post vax, pt stood up and watched sister receive IZ. While watching, pt verbalized to mother that she felt dizzy and fell immediately back and hit head on floor. Syncope. Feet elevated and head lowered after pt passed out and then awoke. Pt verbalized head and neck discomfort. EMS was called.

Write-up: On 3/14/00, the pt had a viral illness with fussiness and discomfort. On that same day, he received Extendryl and Comvax. He takes Tylenol. On 3/14/00, the pt received an injection of Infanrix. On 3/14/00, the pt experienced constant irritability, a worsening of his baseline condition, after receiving his immunizations. The pt developed a fever greater than 104 degrees rectally. The pt was seen in a clinic as a first time pt on the day of admission (3/15/00) with extreme excessive irritability, extremely difficult to console. At that time, the fever was greater than 104. Due to the excessive irritability and marked febrile illness, the pt was admitted for observation and further care. At the time of admission, temp was 103/3, pulse 136, respiratory rate 40 even and unlabored. He was discharged on 3/17/00 with final diagnoses of irritability and reaction to DTaP. He was treated with IV fluids and Tylenol. further treatment of pt during hospitalization is unknown as hospital discharge summary was not provided. The most recent information received 4/27/00 reports the condition of the pt as fully recovered.

Write-up: The reporter received a telephone call from a medical examiner who indicated that on 4/9/00, the pt died of a presumed SIDS. History that the medical examiner obtained from the mother indicated that she and the baby had gone to bed about 3:00am which was not unusual for the baby. The mother slept in the bed with the baby until about 2:00pm when mom awoke. She found the baby dead. The baby was sleeping on his tummy, which has been her practice, as she indicated to the medical examiner that the baby seemed to sleep better and longer and fussed less when he slept on his tummy. In addition, the medical examiner notes that this was a soft bed with a depression in the middle with quite a bit of bed clothes etc, including an electric blanket. He indicates that the entire family were heavy smokers. Medical examiner did not see any evidence of foul play. He is signing this out as a SIDS.

Write-up: Post vax, pt experienced altered mental status per parents. Pt was admitted with possible encephalopathy associated with the vaccine. Per discharge summary pt went to sleep after leaving the office (from receiving vax) and had not awakened except briefly and incompletely to feed. She had not let anyone know of this previously. No vomiting, diarrhea, congestion, coughing, or fever.

Write-up: Mother stated to MD that the baby was fed formula and at 3:00am was returned to his crib. Baby seems to be sleeping later than usual and when mother checked infant at approximately 8:00am he was on his back and unresponsive. Per mothers report, infant experienced diarrhea, nasal congestion, no fever was noted. Autopsy states cause of death as interstital pneumonitis. Autopsy shows also pulmonary edema, focal intrapulmonic hemorrhage.

Write-up: Found apneic and asystolic, tied up in blankets 24 hours after vax. Autopsy shows final cause of death as sudden infant death syndrome. A follow up report received 6/12/2000 clarifies that pt was found "tangled up in his blankets" rather than "tied up in blankets".

Motrin given for fever which was up to 104 per ear probe. Child taken to hospital. Blood work, urine, and observation conducted.
