Write-up: From telephone contact with reporting physician, it was stated that an 18 year old female pt received a Menomune A/C/Y/W-135 vaccination on 6/27/01. Twelve days later, the pt complained of severe headache. She was hospitalized on 7/9/01. The pt is now at home and the headache has completely resolved.

Write-up: Developed markedly red, indurated, swollen and painful left upper arm (elbow to shoulder) with fever, approx. 18 hours, post vax. Reaction subsided very slowly over approx. 8 days time.

Write-up: It was reported that a 19 year old female pt was vaccinated in September 2001 with Menomune. The pt was hospitalized on 04/23/2002 with fever of 101 deg. F., chills, nausea, and petechiae. Negative for headache and stiff neck. Initial diagnosis septicemia. No lumbar puncture performed. Blood cultures positive for Neisseria meningitidis sero group C. The pt was treated with Rocephin and discharged on 04/29/2002. Final diagnosis Meningococcaemia. From the additional correspondence received stated that the pt was admitted to the hospital via the emergency roon on 04/23/2002 following a brief illness in which she is experiencing high fever, body aches, and a rash. "She underwent a complete septic work-up, including urine, blood and throat cultures. After 48 hours, the blood cultures returned as Neisseria meninitidis. The following day, type specific was obtained and it was found to be Type C. At the time of admission, she was begun on Rocepin, and defervesced within 24 hrs from the beginning of antibiotics and over the course of her hospitalization, her rash resolved other than the purpura. There were no coomplications. She received no additional therapy and no further consultations were obtained." The pt was discharged on 04/29/2002.

Write-up: From correspondence received at manufacturer on 15 MAY 2002 it was reported that a 24 year old female patient received a Menomune A/C/Y/W-135 vaccination on 19 NOV 1995. Reportedly, the patient complained of a headache for approximately 2 days prior to 1 MAR 2000. The patient presented to the hospital complaining of headache, initially sent home. The patient went back to the hospital later that evening. Patient was diagnosed with bleeding brain stem, suffered right side paralysis, was unable to speak or swallow. The patient recovered spending approximately 2 months in rehab center. The follow up received on 5/30/02 states call to RN to verify hospitalization. Pt was in ICU on a ventilator for over a week. AFter hospitalization was transferred to a rehabilitation center. Pt stated she had regained most of her strength back. She had called to ask if there were any other cases similar to hers. She inquired about what preservative is used in this vaccine, and if thimerosal was used. Was planning to discuss this with her neurosurgeon. From additional information, it was reported that the patient''s current medical condition is unknown. From additional information received on 05May2004 from a health care professional, it was reported that there has been no contact with the pt since the event of hemorrhage occurred. No further information is anticipated, this case is considered closed.

Write-up: Petechaie erythematous rash on lower extremities first seen on 05/22/02, some flat lesions, some raised without discomfort-dermatology referral; seen 05/22/02. Rash increased when seen 06/03/02 -using topical steroids area of rash increasing up legs and elbows slight edema of elbows, complaint of ankle/elbow pain, also large amount of blood in urinalysis. 06/04/02 admitted by rheumatologist , no steroids and diagnosis of H-S-purpura. Follow-up information: Ulcerated lesions of ankles, vesiculated lesions 1-2 cm of ankles. Palpable purpura of ankles and feet. Large 5-10mm flat macules of legs. Smaller 2-5mm flat maccules of hands and forearms. Patient received Meningococcal vaccine on 05/07/2002. 10 days later developed a palpable purpuric rash on her ankles. She went to a dermatologist who gave her hydrocortisone cream. Over the next several days the rash spread up her legs and she developed-ankle knee, and elbow swelling. Referred to rheumatology and performed a skin biopsy which revealed Leucoclastic vasculitis. She was given diagnosis of HSP and admitted for evaluation. On the day of admit she began complaining of epigastric abdominal pain and blood was seen on U/A.
