Write-up: Two hours after vax, pt was unresponsive and mildly cyanotic. Eventually he was able to be aroused but was irritable when he awoke. Normal the next day.

Write-up: Post vax, he vomited several times and developed a temp of 102-103. She gave him a minimal amount of Tylenol, however, she noticed that he was walking unsteady. On exam, pt''s nose was full of green mucus, his left ear was full of wax. His pulse was 130. He indicated tenderness in all quadrants of his abdomen but there was no reflex guarding or any sign of objective tenderness. However, again in the ER, he did vomit once. I think the pt is having a reaction to his IZ, more probably the DPT than anything else. He did get the shot polio, not the live oral one. However, mom was quite concerned and because this reaction seemed a little more severe than others, I decided to keep him in observation.

Write-up: Information has been received from a LPN concerning a normal, healthy 13 month old female child with no significant past medical history, who on 3/7/2000 vaccines. Post vax, on 3/11/2000, the pt experienced a seizure (considered an other medical event). the pt sought unspecified medical attention. Follow-up information received from a LPN stated that as far as she knows, the child did not have any fever preceding or around the time of the seizure, so she does not believe this was a febrile seizure. The reporter indicated that the child has recovered from her seizure and has had no further seizure activity or after effects from the event. Follow up on 12/06/2000: "Follow up information received on 04/28/2000 from a physician indicated that the patient had 3 seizures on 03/11/2000, required an emergency room/doctor visit, and was hospitalized for two days. The physician reported that the magnetic resonance imaging (MRI) and head computed axial tomography test results were normal and that on 03/12/2000, the patient recovered. No further information is available."

Write-up: On 3/3/00, this 5 month old male had tonic clonic seizures. It was witnessed by the ER physician. The child was given Ativan and the seizure stopped. The child had a low grade temperature of 100.6. The child has received immunizations on 3/1/00. The pt was treated with antibiotics for 48 hours and was discharged on Phenobarbital and Ceclor.

Write-up: It was reported that a 3 month old female, sometime post vax, developed cellulitis of her left leg. The pt was hospitalized for 36 hours (April 12th-15th). Hospitalization time and the lot # provided are to be clarified. Follow-up received on 5/15/00, reported that the pt recovered from this experience. No further information is expected, this case is closed.
