Write-up: A 2 month old pt received Menigitec (meningococcal group C conjugate vaccine, Hib, DTP and Poliomyelitis virus vaccines and 1 day post vax, the infant died. Postmortem result was determined to be sudden infant death syndrome. The pt was born premature at 31 weeks gestation and has a history of intrauterine growth retardation. The reporter noted that the infant was well the day prior to immunization. Sudden infant death syndrome is not expected per the investigator brochure.

Write-up: On 8/2/00, pt developed an increased frequency and severity of seizures with a prolonged apneic phase. The pt was treated with loading and maintenance doses of phenytoin and phenobarbitone, however, he died that day. The possible cause of death was listed as convulsions aggravated. Condition aggravated, epilepsy aggravated and apneic attack are unexpected per the Investigator''s Brochure.

Write-up: Initial information received on 29-Mar-2019 regarding an unsolicited valid serious case received from a consumer/non-healthcare professional by phone. This case involves a 19 years old female patient who died suddenly from meningitis, while received vaccine MENINGOCOCCAL VACCINE. The patient''s past medical history, medical treatment(s), vaccination(s) and family history were not provided. Concomitant medications were not reported. On an unknown date, the patient received a dose of suspect MENINGOCOCCAL VACCINE produced by unknown manufacturer (lot number, expiry date, dose, dose in series, route and site of administration were not reported). On an unknown date, the patient developed a serious meningitis (meningitis) (reported as serogroup A), following the administration of MENINGOCOCCAL VACCINE. This event was leading to death. It was reported that the patient had been given the meningococcal vaccine but still contracted the deadly disease. It was also a case of suspected vaccination failure. No lab data was reported. Final diagnosis was (fatal) died suddenly from meningitis. It was not reported if the patient received a corrective treatment. The Patient died on an unknown date from meningitis. Patient reportedly had received meningococcal vaccine for which the name and manufacturer were unknown. Write-up: The patient was visiting, where she received the Men B vaccine about 2 weeks before presenting to our hospital gravely ill. Within days of receiving the vaccine she developed malaise. A few days before presenting to our hospital she developed subjective fevers and worsening malaise. On the day of presentation she had persistent vomiting, then had a seizure (no history of seizures) that was treated with anti-epileptics at an outside hospital. She was transferred to our institution and, en route, developed agonal breathing and abnormal posturing, with uncal herniation found on head CT soon after arrival. She received ceftriaxone and acyclovir and underwent external ventricular drain placement, but was declared brain dead <24 hours after arrival at our hospital. Autopsy is underway, but no causative agent has been identified yet. - Note: As the MenB vaccine was given somewhere else, we do not know the exact date / details of the vaccine (date listed in this form is estimated, as the family stated it was given 2 weeks before she presented to our hospital on 7/4/19)

Write-up: side effects; This case was reported by a consumer via interactive digital media and described the occurrence of adverse event in a female patient who received Men B NVS (Bexsero) for prophylaxis. On an unknown date, the patient received Bexsero at an unknown dose. On an unknown date, less than a day after receiving Bexsero, the patient experienced adverse event (serious criteria death). On an unknown date, the outcome of the adverse event was fatal. The reported cause of death was adverse event. It was unknown if the reporter considered the adverse event to be related to Bexsero. Additional details were reported as follows: This case received from media. The reporter knew someone who''s daughter took the vaccine and died within 24 hours from side effects.
