Write-up: A physician reported that a 43 year old female inadvertently received an injection of Diphtheria and Tetanus toxoids adsorbed for pediatric use on 3/3/00 and developed a fever of 105, arm swelling and "got sick." She was seen at the ER on 3/6/00. A dx of cellulitis was made and she was treated with IV Rocephin. The pt was later admitted. White blood cell count was 10,800 and x-ray finding were negative for gas or foreign bodies. She was treated with Vancomycin. The pt was discharged 3/9/00 on Keflex and Motrin or Tylenol for fever. At home she continued to have low grade fevers and chills. On 3/15/00 she was seen in the emergency room for a follow-up visit. Her left bicep was characterized by a 10-cm area of tenderness induration and warmth. Blood pressure was 110/90, pulse 99 and respiratory rate 16. Blood cultures and CBC were ordered, results not provided. This is one of five patients from this facility who inadvertently received Diphtheria and Tetanus toxoids adsorbed for Pediatric use lot 462-357.

Write-up: A nurse reported that a 54 year old male received TD Absorbed for wound prophylaxis of a thigh laceration on 3/20/2000. On 3/21/2000, he developed atrial fibrillation, syncope, low grade fever and hypotension. He was hospitalized.

Write-up: Left thigh edematous. Cellulitis status post immunizations. The child was treated in the hospital with IV antibiotics. Upon discharge, the area was slightly red and swelling was still present on left thigh.

Write-up: No adverse reaction to immunizations. Unknown states "as far as we know, pt did not have reaction (fever etc). Infant given a bottle and put down for nap. Later the babysitter found the baby unresponsive. Autospy states cause of death as pneumonia. Autopsy also shows pulmonary edema and congestion.

Write-up: Before leaving the clinic, the pt fainted and collapsed, falling backwards onto the floor. The pt was lying on the floor with symptoms of a seizure. Trembling and twitching were occurring of the hands and neck. The pt''s face was rigid and both eyes were open. The duration of this episode was approximately 1 minute. Subsequently, the pt became combative (kept trying to get up), belligerent, and displayed disorientation as she did not recall getting vaccinations or where she was. Blood pressure was 130/80, pulse 100 and regular, pupils equal, and skin was warm and dry. Pt had a complaint of a headache. The pt was detained for approximately 15 minutes until symptoms improved. The pt left will all symptoms gone except the belligerent attitude. The parent was compliant with referral to the hospital emergency room so the pt may be examined by a physician. Following the exam the pt was sent home. The pt was doing well until about 10:00 PM when she started vomiting and had a complaint of a headache. She was taken back to the ER assessed and transported to a hospital in another city. It was reported she had surgery to relieve inter-cranial pressure. On 1/25/00, the pts father reported she was doing "OK" since the surgery.

Write-up: 6 days post vax, pt experienced Petit mal seizures, 2 one at 8:45 pm; 45 mins after waking up and one at 2:00 pm;45 mins after waking up. Pt was hospitalized overnight for seizures. Pt developed a fever of 102-103 and lasting from 2/11/00 through 2/13/00.

Write-up: Discharge dx: fever secondary to IZ. Discharge meds: tylenol prn. Approx 3-4 hours after receiving vaccines developed fever and brought to the ER with some irritability. Given a shot of IM Rocephin because of concern over possible sepsis.
