Write-up: Pt did not respond when trying to wake her the AM of 2/18/00. Pt was cold, clammy, shallow breathing, eyes rolled back into head, lethargic. Dx: Obtunded & admitted to ER. Given IV & glucose solution. Blood sugar was 38 & keytones high. FU received notes that pt "has been recently dxed with ketotic hypoglycemia - this appears to be the cause of the events in question." Annual follow-up states the pt was hypoglycemic and it does not appear that the reaction was due to the vaccine.

Write-up: Pt became limp, pallor, diaphorectic & had low grade temp 100.3 at ER. Severe symptoms lasted 10 minutes. No further espisodes reported during hospital stay.60 day follow-up dated 06/29/00 provided no additional data.

Write-up: On 2/2/00, pt recv''d MMR,Varicella, Hib, & IPV. On 2/10/00, pt recv''d DTaP. Within 1 hr, pt beame fussy & dvlped fever.He also began vomiting, dvlped lethargy and some "jerking".Also noticed hive-like spots around DTaP inject site. Follow up information states that child had loose cough for about 1 month post episode and rash for 6-8 weeks. Now resolved. 60 day follow-up dated 07/24/00 provided no additional data.

Write-up: Post vax, the pt became extremely weak and muscles were stiff. She struggled for a two week time frame dealing with this problem. Myalgia - Complained of soreness in left arm, shoulder pain, soreness in groin and hips, low grade fever. She was hospitalized and started on prednisone with dramatic improvement.

Write-up: Registered notified by HCA that she heard client had reaction to shots. Registered nurse called mother and she reports client did have a problem post shots and apologies for not alerting agency. She does have copies of paperwork and will bring to agency. Mother reports one week post vax, ran 104 temp. This lasted for 1.5 weeks. Two weeks post vax, client started having bowel troubles and was taken to emergency room with possible bowel obstruction. Two weeks later, severe bruising noted on foot, waist, groin, where anything was tight on skin. On 02/04, client was taken to hospital with low platelets. Was negative for cancer and mom states doctor is not sure if shots were cause of idiopathic thrombocytopenic purpura, but could be. Client is currently recovering.

Write-up: Staring spell and unresponsiveness for about 5 minutes, subsequently, rhythmic twitching of fingers during sleep for a few minutes. (Diagnoses as at least two complex partial seizures by pediatric neurologist.) During the following week, stomach upset, pain, vomiting, fever, diarrhea.

Write-up: Pt received immunization at MD''s office and then went home. Pt was sleeping later when mom heard screaming. Mom went into pt''s room and pt was screaming for a few minutes. Pt then went limp and pale in moms arms and wasn''t responding for several minutes. EMS was called, upon arrival, pulse ox 85.5. Given 15L02 by non-rebreather and taken to ED. Admitted to hospital. Admitted for evaluation. Thought to be hyporesponsive hypotonic episode.
