Preexisting Conditions: Her medical history includes a long psychiatric history with anxiety and panic disorder, posttraumatic stress disorder and attachment disorder. No known medication allergies. The family history is not known. She is adopted. She has been adopted since age 5. No history of alcohol or tobacco. Records received 5/16/07- HX of anxiety, panic disorder, post traumatic stress disorder. HX shingles, attachment disorder.
Allergies:
Diagnostic Lab Data: Her blood pressure is 118/79, temperature 36.7, pulse 81 and respirations 22. HEAD, EARS, EYES, NOSE AND THROAT Unremarkable. Conjunctivae are pink. Buccal mucosa are moist. No cervical lymphadenopathy. Thyroid not palpable. CHEST Clear to auscultation and percussion. No wheezing. Regular.Ther is a grade 2/6 systolic murmur at the left sternal edge. ABDOMEN Soft. There is tenderness in the right lower quadrant and right periumbilical area. There is no guarding. Bowel sounds are present and normal. She has no edema. No cyanosis or clubbing. Feet are warm. No calf tenderness. Homans is negative. She is alert. She is oriented. She has fairly good strength bilaterally. Her white count is normal. Urinary exam is unremarkable. The electrolytes are also normal. Upper GI series unremarkable except for a mild reflux. Small bowel follow through no definite small bowel abnormalities seen. Pelvic sonogram normal. Gallbladder sonogram mild dilatation of common bile duct. no elevated white count.

Write-up: A 17-year-old female who was brought to the emergency room because of progressively worsening right-sided abdominal pain since the day before her admission. She developed some nausea and had emesis in the emergency room. No previous history of abdominal pain. Additionally, she had received an injection of Gardasil three to four days prior to onset of these symptoms. She has no fever and no chills, No chest pain. Nausea and vomiting, as stated. No diarrhea. She did have a normal bowel movement yesterday. Her periods had been regular until she was started on Depo-Provera. No fever. No chills. No significant weight changes. No history of diabetes. Mental health problems as stated earlier. 05/16/07-records received for DOS 3/22-3/30/07;DC DX:Abdominal pain, question etiology. Possible GE reflux disease. Gastritis. post traumatic stress disorder. Question biliary colic. Dilated common bile duct, possibly normal variant. Admitted from ED with worsening right sided abdominal pain. Nausea and emesis. No fevers,

Subsequently, "within one week of vaccination" the patient developed extreme anxiety, sleeplessness and was in a "bad psychological state". Write-up: Information has been received from a nurse concerning a 15 year old female who on 16-MAY-2007 was vaccinated with a first dose of Gardasil (lot 657622/0388U). On 16-MAY-2007, the patient was concomitantly vaccinated with hepatitis A vaccine (inactive) (manufacturer unknown). The nurse reported that the patient stated that she received a second dose of Gardasil, but the nurse noted that there was no documentation that the patient received a second dose of Gardasil. On 23-AUG-2007 the patient experienced fatigue, altered mental status and weakness. On 25-AUG-2007, the patient was brought to the emergency room. The patient was hospitalized (details and length of hospitalization not provided). The patient was visited by her physician on 25-AUG-2007, 26-AUG-2007, and 28-AUG-2007. At the time of this report, the outcome was unknown. Additional information has been requested.

Write-up: Received HPV #1 vaccine 9/13/07. Mother called on 10/29/07 stating since vaccine given, child''s "mental state altered greatly. All of a sudden she became this witch." Since then "she became very aggressive and almost pouty in every situation". She states that the child hit her (the mother) last Wednesday or Thursday. Mother states that since the shot the child "has become aggressive, dark, argumentative". She further states that since the shot the child has become "mouthy to the teachers". She has always been a "spirited child" according to the mother. Mother now states that the child seems to be calming down some. I offered to submit a VAERS report and mother agreed that I should do so.

Write-up: Information has been received from a medical assistant concerning a female in her early 20''s who on an unspecified date in 2007 the patient received her first dose of GARDASIL injection. Concomitant therapy on the same date also included another vaccine (therapy unspecified). On an unspecified date the patient received her second dose of GARDASIL. The patient had a "nervous breakdown; some kind of anxiety or severe depression". The patient was admitted to the hospital and was found to have decreased serotonin levels and aluminum toxicity. The dates of the hospitalization are not known but the patient was discharged and was started on an antidepressant (therapy unspecified). The patient has not received her third dose yet. At the time of reporting the patient was recovering. Additional information has been requested.

Write-up: Information has been received from a physician''s assistant, via a company representative, concerning an 18 year old female patient, the daughter of one of her patients, who on an unspecified date was vaccinated with the first dose of Gardasil (lot # not reported). The PA stated the patient had been a normal 18 year old, who immediately after the vaccination, "started acting very strange, almost like she was on drugs, depressed." She added the patient "started to have mental illness, drug-like symptoms." The patient was taken to a specialist, who told her she had abnormally high levels of aluminum in her body as a result of receiving the Gardasil. The specialist felt that the symptoms were caused by the aluminum. At the time of this report, the outcome of the events was not specified. Additional information has been requested.

Diagnostic Lab Data: LABS: MRI of head & spine WNL. CT angiogram of head WNL. PET scan abnormal. Right side craniotomy for biopsy of right frontal lobe. EEGs slow but WNL. CSF: WBC 14 monos, protein 17, glucose 68, cytopathlogy revealed mostly normal lymphocytes, mostly Tcells. DNASE-B AB 680 (H). ESR 33, CRP 0.2. TTE revealed ejection fraction WNL w/mild LVH. Urine c/s (+) for e. coli. Tracheal aspirate (+) MRSA. All blood c/s neg. Renal CT scan w/lesions.

Write-up: 21 y/o female seen in ER 13 May with C/O it hand & lt. foot numbness approximately 1 month post GARDASIL vaccine. She also C/O of nausea, vomiting and diarrhea. A heat CT demonstrated bilateral basal ganglion infarcts and a MRI showed with similar results with no acute process. She was admitted 20 May with left hemiballsmus. On 22 May her mental status declined. She received many doses of HALDOL and ATIVAN for agitation and was intubated on 28 May. She was transferred to medical center 28 may for w/u for encephalitis and has since become comatose. Symptoms: Nausea w/vomiting, Diarrhea, Nausea w/o vomiting, Numbness. 7/11/08 Reviewed hospital medical records for 6/11-6/23/2008. FINAL DX: aseptic meningo-encephalitis; choreoathetosis r/t meningo-encephalitis; paroxysmal autonomic instability w/dystonia; e. coli UTI. Records reveal pt transferred from military hospital. Military deployment 1-3/2008. On leave 5/2008 developed n/v/d, abd pain & bifrontal HA which continued to worsen. Sought medical help but no dx given. Developed left side weakness, flaccidity & several falls. Found 5/12 unable to ambulate & incontinent of urine. Taken to ER where MRI revealed possible basalganglia infarct. To ICU where subsequent MRIs were WNL. Ventilated & remains in persistent vegetative state. LPs done. PICC , trach & PEG tubes. Intermittently febrile, labile BP, asystole x2. Tx in multiple hospitals w/multiple meds including steroids, IVIG, antibiotics & antivirals. 12/11/08 Reviewed additional medical records of 5/08-7/08. FINAL DX: Records reveal patient experienced nausea/vomiting/diarrhea, dizziness & HA, left hand/foot numbness x 4-5 days. Seen in ER on 5/13 where labs were WNL but CT of brain revealed lacunar infarcts, bilateral basal ganglia & cavum verge. Numbness continued & developed involuntary jerking of LEs & difficulty walking. Admitted to MICU 6/08 w/encephalitis. Tx w/IVIG & steroids. Transferred to higher level of care & then to inpatient rehab program.
