Write-up: 3rd HPV given on 2/1/17, previously healthy. Returned on 3-2-17 with fatigue and dizziness. Seen on 3/7/17 with fatigue, dizziness, decreased appetite. 3-15-17 symptoms increased, exhaustion. 3/24/17 dizzy, tired, hyperacusia, legs shaking.
