Write-up: Information has been received from a certified medical assistant via medical records concerning a 4 year old female who on 28-SEP-1995 was vaccinated with the first dose of MMR II. In approximately 1998 the patient developed chicken pox. The outcome was unknown. On 06-AUG-2007 the patient was vaccinated with the second dose of MMR II. Subsequently, the patient had inappropriate schedule of vaccination of GARDASIL (MSD) (WAES# 0905USA00732). Additional information has been requested.
