CDC 'Split Type':

Write-up:Menactra, Gardasil, Hep A (Vaqta) given 7/13/07. Left here, shortly after felt disjointed and blank stares. At approx 2250 came home found to have tonic-clonic approx 45 sec sz with heavy breathing and frothy. 911 called admitted x 2 nights. 7/19/07-recor"ds received: Seizure likely secondary to vasovagal reaction. After vaccine felt a bit disjointed and had a somewhat blank stare. Clonic movements, snorting and grunting, frothing at mouth. Travel to Caribbean 2 weeks prior. Headache. Neuro exam WNL. Asses
