Write-up: First Gardasil given 2-14-07 pt returned 4-11-07. Informed nurse of episode of dizziness severe with diaphoresis - near syncope - within 3 days of 1st injection - ER visit - DX possible seizure - referred to neurology. Neuro cleared pt feels not related to Gardasil event occurred actually 3wks after injection.
