On 22-MAY-2007, the patient took a pregnancy test in the doctor''s office which was positive. The patient was unsure of the date of her LMP. No problems were reported. Follow up information from the healthcare professional indicated the patient "had decided to terminate the pregnancy," although the reason for the decision was not specified, and the date for the procedure was left blank; it was not confirmed that the procedure had actually been completed. Upon internal review, decided to terminate the pregnancy was determined to be serious as an other important medical event. Additional information has been requested.
