Write-up:Information has been received from a physician''''s assistant and a nurse, concerning the nurse''''s 14 year old daughter, who on 02-FEB-2007, was vaccinated in the right hip, with the first dose, 0.5ml, of Gardasil, and on 04-APR-2007 was vaccinated in the b"ack of her right arm, with the second dose, 0.5ml, of Gardasil.
