Write-up: Flu like symptoms after 2nd Gardasil injection 10/21/08 Reviewed endocrine clinic records of 2/10/06-3/11/2008. FINAL DX: Hashimoto''s thyroiditis, hypothyroidism, dysfunctional uterine bleeding, poor circulation in feet Records reveal patient experienced rapid weight gain, irregular periods, dysfunctional uterine bleeding. Tx w/thyroxine replacement therapy (synthroid). Dx w/Hashimoto''s thyroiditis w/hypothyroidism 2/2006. 10/10/08 Reviewed OB/GYN notes of 1/11/06-8/26/2008. FINAL DX: goiter, dysfunctional uterine bleeding, Records reveal thyroid enlarged. Referred to endocrinologist. 4/19/07 parent called office w/reports of body aches, dizziness, chest congestion, HA starting after 2nd HPV vaccine on 4/7/07. Improved w/NSAIDS.
