Write-up: Received Gardasil on 03/09/2007. Patient reported numbness / tingling feeling in left fingers, arm and left swollen lip, then hands felt floppy / numb. Negative for shortness of breath, chest pain, fever, injury. Physical exam grossly normal, normal neurological exam.
