Write-up:Initial and follow-up information has been received from a physician concerning a female who was vaccinated with a 0.5 ml dose of Gardasil (lot #655617/1447F). Subsequently, 2 days after vaccination, the patient experienced "RSD reaction" that started in her upper extremities and then moved to her lower extremities. The patient is undergoing a neurology work-up. At the time of the follow-up report, the patient had not recovered. Additional information has been requested. 05/15/07-records received for DOS-1/15/07-seen for repeat chlamydia testing after treatment. C/O episodes of chest tightness without SOB and is worried about pulmonary embolus after reading about this risk on the internet. NuvaRing prescribed. Letter from parent included in notes-per parent. patient had thrid in a series of 3 shots of B12 on 4/16. Developed paresthesia in arm and fingers curled into her palm. Contacted neurologist who referred her to ER, treated with bendaryl and morphine and her arm straightened. Also previously seeing a physical therapist.
