Write-up:Information has been received from a nurse a the daughter of a female patient (demographics not provided) who on 06-SEP-2006 was vaccinated with the first dose of Gardasil (Lot # not provided). The mother reported to the nurse that on 06-SEP-2006 after re"ceiving the vaccination her daughter developed an elevated temperature (actual measurement not provided), nausea and chest pain.
