Write-up: Information has been received from a physician concerning a female patient who on approximately 16 Nov 2006 was vaccinated with a 0.5ml IM dose of HPV vaccine. Two days later dosage, on approximately 18 Nov 2006 the patient developed a rash at injection site. The patient sought unspecified medical attention. Additional information has been requested. This is in follow-up to report(s) previously submitted on 12/14/2006. The patient was told to take BENADRYL and to make appointment. The patient never kept appointment and the office is "unable to contact her".
