MR received for ER admit for c/o dizziness, vertigo, and vomiting x2 days beginning within 24 hours of Gardasil vax. PE: WNL and in no distress as long as she holds still. Txd with IV fluids, Valium and Antivert. D/Cd home on Meclizine after 48 hours. Discharge DX: Acute vertigo, possible adverse reaction to Gardasil vaccine. 02/05/2007 Tag-2 received. No new codes.
