Write-up: Pt felt dizzy after shot given but then felt better. Went to pharmacy where her vision clouded, has brief clonic jerk of her arms, then started to crumple (was seated in chair). On my arrive pt extremely pale, and pulse quite low. Before could count HR, it accelerated to high 90''s, BP systolic in mid-90''s, and color came back. Came back to pediatrics via wheelchair until felt back to baseline. Orthostatic VS normal at discharge.

Write-up: From initial information received on 6/17/05 from a registered nurse, it was reported that a 17 year old male patient received a dose of Menactra, lot number U1574AA, administered IM in the left deltoid on 6/7/05. Seven days later, on 6/17/05, the patient developed a headache, vomiting, myalgia and a petechial rash over his entire body. The patient was diagnosed with ITP. At the time of this report, the patient had not recovered from these events. (OMIC)

Write-up: Vaccines 06/15/2005 - c/o right deltoid site pain, 06/15/2005 pm. 06/16/ - seen in office with positive gastroemesis, hamstring tenderness. Blood work done to rule out myositis. 06/17 - pain improving, fever 101.7 in am, 97.4 at time of visit, lyme and EBV titers normal was well.
