Write-up: Redness swelling of injection site about 24 hours after injection, 48 hours out has redness, warmth, pain to touch on LA, 2 quarter size areas of red warmth on RA. LA - 5 1/2 x 5 1/4 inch redness.

Write-up: Redness at site of injection. By the next morning progression in redness and started getting harder. Red area about 3 inches in diameter on left back arm. Diagnosed with a cellulitis and was given oral antibiotics.

Write-up: (5-1-12) An hr. after injection was given pt was having pain. The next morning pt woke up (5-2) and had a circular red spot (puffy) where the shot was given. Today -(5-3) still had an larger but no pain "kind of itchy".

Write-up: This case was reported by a physician, via a sales representative, and described the occurrence of amenorrhea in a 14-year-old female subject who was vaccinated with CERVARIX (GlaxoSmithKline). On 19 December 2011 the subject received 1st dose of CERVARIX (details unknown). On 19 December 2011, less than one day after vaccination with CERVARIX, the subject experienced amenorrhea. The physician reported that the subject received her first dose of CERVARIX and had experienced amenorrhea since that time. The vaccination series was delayed. At the time of reporting the event was unresolved. The physician considered the event was possibly related to vaccination with CERVARIX. Follow-up information was received on 27 March 2012. It was reported that CERVARIX was administered intramuscularly in the left arm of the female subject on 19 December 2011. At that time, the subject was having menses. The subject experienced an absence of menses in January 2012 and February 2012. This resolved in March 2012 with the return of menses. In the reporter''s opinion, the event was possibly related to vaccination with CERVARIX. No concurrent medications were noted. The subject returned for her 2nd dose of CERVARIX in March 2012. It was not known if the event recurred.

Write-up: This case was reported by a healthcare professional and described the occurrence of amenorrhea in a 14-year-old female subject who was vaccinated with CERVARIX (GlaxoSmithKline). Concurrent medical conditions included BACTRIM allergy, epigastric pain and sore throat. On 13 April 2011 at 14:30 the subject received 1st dose of CERVARIX (.5 ml, unknown route, right arm). In April 2011, less than one month after vaccination with CERVARIX, the subject experience amenorrhea and on 01 February 2012 experienced an ovarian cyst. The subject was administered the CERVARIX on 13 April 2011. In July 2011, the subject was seen by the healthcare professional and did not mention the amenorrhea. In November 2011, the subject was seen by the healthcare professional and the subject''s mother reported that the subject had not had her period since April 2011. The subject was referred to Obstetrics and Gynecology who found a right ovarian cyst on 01 February 2012 by ultrasound. At the time of reporting the outcome of the events was unspecified. The vaccination course with CERVARIX was discontinued. On 06 March 2012, updated lot number information was received from a safety review. The lot number was reported as AHPV096AD from the reporter; however, based on safety lot number review, the lot was updated to APVA096AD. Follow up information received from the healthcare professional on 14 March 2012. The healthcare professional reported that the subject experienced the events on 13 April 2011 and the events resolved on 05 February 2012. The healthcare professional noted that the events were not serious. According to the healthcare professional, the CERVARIX immunization series has been delayed until the subject''s parent decides to continue.

Write-up: I have on and off fever, vomiting, nausea, chilling. The problem is I am currently spending tourist visa-May 9 to May 16- with my mom and my 3 kids and they trace my 40.3 fever, that time only small cold I have when I enter, I inform them of my hpv vaccine and show when the pack indicating the shot given May 7 night time by my auntie doctor, I trust my doctor auntie because she take also same brand, she is working in dept. of health - CERVARIX by glaxo smith kline-, I am now starting cough -May 10- and continuos on and off chilling 4 to 5 times a days. Now I am depressed, there is no way that I can accompany my kids, this is 1st time for them to go outside the country and happiness experiencing disneyland, I am new and I don''t know where to ask help beside my fund is enough for our trip. Until now may days the side effect I dont know. The happiness with kids is really cannot paid by money, the sadness of them without me is a lifetime bear in their mind that their mom is not with them only their grandmom that also suffers pelvic muscle pain due to aging. Please tell me that what are the medicines I need to take ...I have paracetamol, vit.c. multivitamins and b1-6-12. . .my location is in hotel I am here until May 11, 2011 and will move - I need your urgent reply, I am really afraid. I tick all the box in "B" because I cannot pursue if I will not put a tick. This is bias, only adverse effect supposed to be.

Write-up: Left office - in car on way to school - felt hazy, nauseated and sweaty; passed out and had generalized shaking movements of extremities < 1 min.; sleepy - came around to full consciousness in 2 min. Returned to office; VSS/PE (-)/Pulse OX 95%.

Write-up: Triaged pt. (took vitals) then administered CERVARIX to (L) arm. Pt. was sitting at the time of shot. Pt. indicated her fear of needles so I instructed her to look away and stay seated. Once given I proceeded to fill out physical form. Pt. got dizzy and fell over about a minute after given. Vitals were taken and pt. was released once vitals were normal and Dr. released. Pt. did not fully pass out. Got dizzy and fell down.
