Write-up: Pt received HPV #2. 3 hours later, pt started have difficulty breathing, rash; lip & tongue edema. Seen at ER for anaphylaxis. S/P epinephrine, methylprednisolone, and diphenhydramine. D/C home on oral prednisone and EPIPEN.

Write-up: Pt. received vaccine at approximately 1:10pm on 7-26-12. Pt. reacted to the vaccine 5 minutes later and was feeling dazed, pale, weak. Treatment with epinephrine .2mL. Pt. recovered 20 minutes after injection of epinephrine. Site was the deltoid of the left arm.

Write-up: This spontaneous report as received from an office manager refers to a 14 years old female patient with asthma. The patient was vaccinated with 3 doses of GARDASIL (lot# not reported) on 16-JUN-2009, 19-AUG-2009 and 18-DEC-2009, respectively. Concomitant medications included ZYRTEC, SINGULAIR, PULMICORT, SYMBICORT, VERAMYST and doxycycline. Since receiving GARDASIL, the patient had experienced migraines, fainting spells and could not sleep. The patient was hospitalized at some point but there was no information about the hospitalization or the name of the hospital. The patient contacted the physician. The outcome of migraines, fainting spells and could not sleep was reported as not recovered. The relatedness for migraines, fainting spells and could not sleep was unknown for GARDASIL. Additional information has been requested.

Write-up: Very bad muscle and joint pains, carpel tunnel syndrome kind of pains. Weakness in arms and legs. Then I was checked for Vit D, it was low, then my thyroid started giving up. TSH kept increasing. So doc gave Vita D and Synthroid starting dose 25mcg. There was no relief. Checked a lot of antibodies like ANA and RF factor. None was present. Now after two years I am still struggling with auto immune thyroid disease and myopathies involving arm muscles, joint pains, constipation etc. Need high doses of thyroid medication to feel better. I should have been warned of this risk of developing auto immune diseases with this vaccination.

Write-up: On 1/25/2012, as per mother patient developed a black bruise to the left upper arm where vaccines had been administered that covered most of the entire upper arm. On February 1, 2012, he developed a sore throat and fever that lasted a week. On February 7, 2012 at 11 am he reported to hospital. As per mother, patient was non-responsive and was throwing up blood-tinged saliva. He was flown out due to an internal hemorrhage. He was admitted to hospital at around 3 pm. After admission, he remained unresponsive. As per mother, the internal cerebral hemorrhage was too advanced and the patient was not a candidate for surgery so patient eventually became comatose. He passed away on 2/10/2012 at 2 am. Final diagnosis was cerebral hemorrhage and leukemia. As per parent, the client had no previous history of illnesses at the time of vaccination on 1/25/2012.

Write-up: She had 3 HPV shots (7/29/2010, 1/11/2011, 7/17/2012) each time afterwards had muscle pain, joint pain, was diagnosed with RND one year afterwards, later retracted, diagnosed with dysautonomia 08/10/2013 and is now confined to wheelchair.
