Write-up: Menactra vaccine administered on 6/15/2005 at ~10:30 am. On 6/16/2005 at 7:00 am, patient''s father contacted MD to report that patient had a cough, fever of 102.6F, sore arm, and headache. Patient was monitored at home by family per MD. Given Tylenol and Advil for fever and arm pain. On 6/17/2005 8:00am, MD contacted patient''s father and patient is afebrile, denies headache and sleeping comfortably.

Write-up: Swelling, redness, sore, warmth at injection site and then bruising at injection site. Still slighty swollen today. Saw MD at clinic for follow-up who told pt. her reaction was not unusual. The size was about 6 inches when it was at its largest.

Write-up: Fever within 6 hours (103-104), neck swelled and got sore, headache. 12 hours, rash on neck and face within 4 days . Rash from neck to groin on front of body; upper back also had rash. Face swelled 48 hours after shot.

Write-up: Developed fever and fatigue the evening the shot was given. Also c/o mild intermittent headache. Fever was up to 102 and lasted for 5 days. Self medicated with advil prn. Patient seen today, (6/21/05) doing better with no fever so far. No further treatment given.
