Write-up: Information has been received from a registered nurse concerning a 20 year old female with no relevant medical history who on 19-FEB-2007 was vaccinated intramuscularly with a 0.5 mL first dose of Gardasil (lot 653735/0688F). Concomitant therapy included Ortho Tri-Cyclen Lo. Subsequently, she became pregnant. Date of LMP was 04-APR-2007. The patient subsequently miscarried. The patient was seen in the emergency room on 14-MAY-2007 for the miscarriage. She was not admitted to the hospital. The patient called the office on 17-MAY-2007 to notify the physician of the miscarriage, but had not returned to the office for follow-up visits. Upon internal review, the patient''s spontaneous abortion was considered an other important medical event. Additional information has been requested.

Write-up: Information has been received from a 23 year old sexually active female who on 08-MAY-2007 was vaccinated intramuscularly in the left deltoid with a first dose of Gardasil (657622/0388U). Concomitant therapy included sitagliptin phosphate (MSD). On 14-MAY-2007 the patient experienced fever, chills, excessive swelling, swollen uterus and swollen ovaries, hot flashes, perspiration, excessive swelling, yellow discharge, odor, irritation, and itching. She took an unspecified pill for itching that was for yeast infections and it helped. As of 15-MAY-2007 the patient started with not being able to hold urine for long periods of time and burning. A complete urinalysis was performed revealing cloudy urine with the presence of ketones, occult blood, protein, leukocytes esterase, white blood cells, squamos epithelial cells, and bacteria all consistent with vulvovaginitis and a Urinary Tract Infection. Atthe time of the report the patient had not recovered. Additional information has been requested.

Write-up: Information has been received from a physician concerning a 15 year old female who on 05-APR-2007 was vaccinated with Gardasil (Lot#654702/0011U). On 05-APR-2007 post vaccination the patient developed a red swollen arm. Unspecified medical attention was sought. Subsequently on 09-APR-2007 the patient recovered from the red swollen arm. Additional information has been requested. In follow-up it was reported from a physician concerning a 15 year old female student whose reported weight was 158 pounds and whose reported height was 66 3/4 inches who on 05-APR-2007 was vaccinated intramuscularly into the left arm with the first dose of GARDASIL vaccine (yeast) (Lot #654702/0023U). It was reorted that on 05-APR-2007, the patient developed left shoulder and upper arm swelling and pain which lasted for 3 days. The patient recovered on 09-APR-2007. It was reported that the patient recovered. No additional information is expected.

Write-up: Information has been received from a licensed visiting nurse (LVN) for the pregnancy registry for Gardasil concerning a 20 year old female with abdominal pain and inguinal lymphadenopathy (treated with doxycline therapy) and a history of recurrent upper respiratory sinus infections who on 30-NOV-2006 was vaccinated with the first dose of Gardasil. On 01-FEB-2007, the patient was vaccinated with the second dose of Gardasil. On 09-MAY-2007, the patient was seen in her physician''s office with a complaint of sinus congestion. On this visit, the patient reported that she was pregnant, and had seen her gynecologist one month ago. The patient''s outcome was unknown. Additional information has been requested. This is in follow-up to report(s) previously submitted on 6/14/2007. Follow up phone call information was obtained from a licensed vocational nurse who reported that the patient vaginally delivered a healthy and normal infant, full-term, without complications. No additional information is expected.

Write-up: Information has been received from a medical assistant concerning an 11 year old female who was vaccinated with the first dose of Gardasil. Concomitant therapy included DTaP (unspecified) (+) Menactra, both administered in the opposite arm. It was reported that 20 seconds after vaccination with Gardasil, the patient fainted. The patient recovered after five to ten seconds, although, bumped her head on the floor. X-rays of the patient''s head were taken (results not reported). The outcome of the patient''s head injury was unknown. Additional information has been requested. This is one of two reports received fro the same source. This is in follow-up to report(s) previously submitted on 6/14/2007. Initial and follow-up information has been received from a medical assistant concerning an 11 year old female student with a history of headaches who on 30-MAY-2007 at 1:45 pm was vaccinated in the left arm with the first dose of GARDASIL (LOT# 654885/1424F). Concomitant therapy included INFANRIX (Lot # U1986AA) and MENACTRA (Lot # U2142AA), both administered in the opposite arm at 1:45pm. It was reported that the patient sat down for5-10 minutes after the vaccinations. When she got up, "she gave about 10 steps and fainted. She hit herself on the head." The patient was observed and a head computed axial scan was performed. The result was normal and showed no infarct, mass or bleed. The outcome of the patient''s head injury was unknown. This in one of two reports received from the same source. Additional information is not expected.

Write-up: Information has been received from a licensed practical nurse concerning a female patient who was vaccinated with a first dose of Gardasil. Concomitant therapy given on the same day included a dose of Varivax and a dose of Tdap on the same day. It was reported that Gardasil was the last in that series of vaccines the patient received. Subsequently, the patient fainted. Unspecified medical attention was sought. On an unknown date, the patient recovered. No product quality complaint was involved. Additional information has been requested. This is in follow-up to report(s) previously submitted on 6/14/2007. Information has been received from a registered nurse concerning a 12 year old female patient with a history of vaginal birth (no complication) who on 09-FEB-2007 at 09:54 was vaccinated IM into the right deltoid with a first dose of GARDASIL (Lot# 654389/0961F). Concomitant therapy given on the same day included a dose of VARIVAX and a dose of Tdap (unspecified) on the same day. It was reported that GARDASIL was the last in that series of vaccines the patient received. After immunization at 09:54, the patient stated the last shot on the right arm was painful. The patient turned pale, went back and hit her head to the wall. The patient stiffened, made some gurgling noise and had her eyes open. It was reported that the patient had a brief episode of fainting on the exam table. It lasted 1-2 seconds. The patient recovered spontaneously. Once the patient came around the patient asked "what happened?" Laboratory diagnostic studies included a "neuro" check. It was reported that the patient was alert to her name, place and was responding appropriately to questions. The patient''s vital signs included a blood pressure measurement of 90/60 and a heart rate of 90. The physician was notified, water was offered and tolerated well. The physician examined the patient. The patient walked out of the room without any difficulty. The physician presumed the experience was transient vasovagal episode. The patient was offered an appointment to return to the clinic only if the patient was up to it. At the time of this report, the patient''s outcome to the vasovagal episode, turned pale and painful right arm was unknown, however, the patient recovered from fainted. No product quality complaint was involved. Additional information is not expected.

Write-up: Information has been received from a medical assistant concerning a 9 year old female who was vaccinated with the first dose of Gardasil. It was reported that the patient appeared to be nervous prior to vaccination, and subsequently, 5-10 seconds after vaccination the patient fainted. The patient''s mother caught her so that she did not fall and incur any additional injury. The patient sought unspecified medical attention. Write-up: Information has been received from a Licensed Visiting Nurse (LVN) concerning a 22 year old female patient with a history of toe surgery who on 22-MAY-2007 was vaccinated IM in deltoid with a dose of Gardasil, lot #657736/0389U. Concomitant therapy included escitalopram oxalate (LEXAPRO) and a multivitamin. On 22-MAY-2007 the patient developed bilateral hands and upper arms reddish-brown discoloration, swelling and pain. She was examined in the office on 24-MAY-2007. Azithromycin (Z-PAK) was prescribed, "not sure if started yet". Her outcome was unknown. It was noted that urine and CBC were taken on 22-MAY-2007, before vaccination, and that neutrophils, MCV (mean corpuscular volume) and MCH (mean corpuscular hemoglobin) were "a little high". Additional information has been requested. Follow up information has been received from the LVN concering the 23 year old female patient with no known drug allergies who on 22-MAY-2007 at 2:00 p.m. was vaccinated IM in right deltoid witha first dose of GARDASIL, lot #657736/0389U. At 9:00 p.m. that evening her hands and feet were red and swollen, painful and itching. One week later, her hands and feet started peeling. The patient recovered on 22-JUN-2007.
