Write-up: Mom states pt came home at 7:00 from date not feeling well. Temp 100 F. Took ibuprofen. At 10:00 began to mumble and moan. c/o SOB. Then started twitching all over- 911 called. Pt taken to nearest hospital. Pt given IV fluids- Tordal and Ativan.

Write-up: Complaint of pain when vaccine given and states arm hurt from vaccine sight (left deltoid) to elbow. Hurts when raises arm. No swelling - redness or atrophy of muscle. Called on 06/10 to report. Talked again with patient on 06/13/2005 and she says it is getting better. Advised to let us know if worsens or need to see PMD.

Write-up: Immunization given on 06/13/2005 at approximately 3PM. At 6:30PM developed temperature 101, weakness and by next day pain to left arm - arm hot to touch, dizzy, pale when got up, nausea. By 06/14/2005 PM was feeling better.

Write-up: Received vaccine 9 AM 6/10/05. 6/11/05, developed fever 101-102, severe myalgias, headache. Responded to Tylenol over 24-36 hours. Known adverse effect but was severe.
