Write-up: Prior to and during vaccine administration, patient was crying and hyperventilating. After vaccines administered, nursing notified that she was not acting appropriately. She was sleepy, difficult to arouse. She was starting off, not responding to questions and difficult to keep awake. Later complaining of chest pain, headache, flank pain, difficulty raising her arm and continues to be slow to answer questioning, not oriented to time or place. Altered mental status of 45 minutes in duration. Glucose normal, given juice and crackers. Tylenol for pain. Admitted to the hospital with neurology consult, and with EKG.

Write-up: Approximately 60-90 seconds after receiving HPV vaccine patient had an episode of syncope. Patient was standing behind her mother, who was seated, and fell to the floor. Patient''s head made an audible "thud" when it hit the floor. It appeared she struck the back of her head. Initially patient''s eyes were rolled back and patient was twitching slightly. She then closed her eyes in an apparent loss of consciousness for 20-30 seconds. Patient did not respond to questions or commands during this episode. Approximately 90-120 seconds after start of episode patient became more alert and began sobbing. Emergency Department was called immediately and 2 ED RNs responded. Pt was transferred (wouldn''t/couldn''t stand) to wheel chair and taken to ED. Patient had received intranasal flu vaccine ~5 minutes prior to HPV. Patient''s mother and 1 sibling witnessed event. Father and 2 other siblings were called to room by mother while patient was still on the ground. Patient was examined and observed in the Emergency Department for 4 hours. She was cleared for discharge to home in parents'' care. Two or three days after event patient presented to outside hospital with reported altered mental status. Patient was found to have small subdural hematoma and was transferred to another hospital.

Write-up: Her symptoms began with dizziness in the morning. The following week she developed an ongoing headache with extreme fatigue. I took her back and forth to the doctor told she needed more sleep and water. Her next symptom was shortness of breath and fast heart rate with increased headache. We were sent to a cardiologist who then sent us to a neurologist. After ER visits, rheumatology, endocrinology, and two more neurologist we still had no help for my daughter who cannot think anymore her muscle and joint pain is crippling and her head hurts so bad she cannot open her eyes. The symptoms just keep coming we are not faced with gastric issues adding to her pain. We received a POTS diagnosis from two neurologists. My daughter cannot play, read, think, stand up, take a shower. She just lays and cries all day every day. My straight A student is now a shell of who she used to be. In pain and not able to care for her basic needs.

Write-up: Information has been received from a consumer referring to her 16 years old daughter. The patient was not pregnant. The patient had no medical history and drug reactions or allergies. On an unknown date in 2011, the patient was vaccinated with her first dose of GARDASIL (0.5 ml, lot # and expiry date unknown, intramuscular). There was no concomitant therapy reported. In 2011, almost immediately following her first intramuscular injection, the patient experienced grand mal seizures. The patient''s mother also noticed at that time that her daughter''s mental acumen began declining (The patient was previously an honor student in school). On unknown dates, the patient was vaccinated with her second and third dose of GARDASIL (0.5 ml, lot # and expiry date unknown, intramuscular). On an unknown date, the patient''s mother took the patient to a neurologist who ran a battery of tests including electroencephalogram (EEG), nuclear magnetic resonance imaging (MRI) and computerised tomogram (CAT scan). Subsequently, the test results determined the patient was "mentally challenged". In September 2014, the patient was hospitalized at a hospital for 2 days due to severe seizures (started in September 2014). The patient was also prescribed TEGRETOL (generic) by the neurologist to lessen seizures. The patient''s mother also stated that the patient was still suffering from severe seizures. The outcome of grand mal seizures, mental acumen decline and "mentally challenged" was unknown. Causality was not provided. The events were considered to be disabling by the reporter. Upon internal review, the severe seizures and grand mal seizures were determined to be medically significant. Additional information has been requested.

Write-up: This spontaneous report was received from a consumer and refers to an unspecified number of male patients of unknown ages. No medical history or concurrent conditions were reported. On unknown dates, the patients were vaccinated with GARDASIL (dose, dosage number, route of administration, anatomical location, lot # and expiration date were not reported). No concomitant medications were reported. On unknown dates, the patients were presented with "new physical and psychological problems". The patients, reportedly, could not perform sexually, with some describing the problem as "something is wrong down there." The reporter went further on to state that such patients needed help in order to cope with a dysfunctional male organ and/or a swollen or painful scrotum. Moreover, the embarrassment was given a note, when the patients could no longer achieve an erection or hold their bladder. The outcomes of the aforementioned events were unknown. The reporter did not explicitly state any causal relationship between the events and vaccination with the suspect vaccine with a notable exception to the event of male sexual dysfunction, which was considered to be related since it was believed that impotency was associated with GARDASIL. Write-up: Rashes from head to toe, fever, hyper-sensitivity to noise, severe insomnia, severe mental degredation/confusion. Episodes are recurring and last nearly a week long each episode. She just finished her third episode. Slept less than 5 hours in 5 days. No appetite. Complete mental shut down like she is drugged. Terrible.

Write-up: Information was received from a lawyer regarding a case in litigation regarding an 11 year old female with history of a twisted and necrotic ovary with surgery (January 2011). It was reported that the patient followed her physician''s advice and on 30-MAR-2011 was vaccinated with her first dose of GARDASIL, (Lot number 667866/1437Z, expiration 25-FEB-2013). On 08-AUG-2011, the patient was vaccinated with her second dose of GARDASIL, (Lot number 667194/1271Z expiration 11-DEC-2012). On 12-DEC-2011, the patient was vaccinated with her third dose of GARDASIL, (Lot number 670049/1261AA, expiration 11-NOV-2013). It was reported that on 30-MAR-2011, almost immediately after the first injection, the patient began to experience stomach pain, bloating, feeling full, nauseated and bladder infection-like symptoms. After the completion of the three vaccinations, the symptoms worsened, and she began to suffer additional symptoms of insomnia, night sweats, day sweats, and chills. On an unknown date, she developed headaches, fatigue, fever, light sensitivity, blurred vision, personality changes, back pain itchiness, dizziness, chest pains and racing heart, spasms in her limbs, tingling and numbness in the hand and feet, confusion, and other mental abnormalities, all of which were reported to be caused by GARDASIL. The patient was told for over three years by her physician that her symptoms she experienced were post-operative symptoms. The patient''s symptoms worsened and medical advice from another physician on 20-NOV-2014, reported that the patient "is vaccine injured". The patient''s mother found medical literature regarding a finding of contamination of GARDASIL in nine different countries, and unfortunately, one of the listed GARDASIL contaminated lots was GARDASIL Lot # 1437Z, which is the lot number of the patient''s first injection of GARDASIL on 30-MAR-2011. The patient''s conditions were reported directly and proximately caused by the GARDASIL and are permanent. The conditions the patient has experienced has continued for over five years and are presumed to be permanent. Stomach pain, bloating/feeling full, nauseated and bladder infection-like symptoms, insomnia, night sweats, day sweats, chills, headaches, fatigue, fever, light sensitivity, blurred vision, personality changes, back pain itchiness, dizziness, chest pain and racing heart, spasms in her limbs, tingling and numbness in the hand and feet, confusion, and other mental abnormalities are considered disabling. Upon internal review, mental abnormalities was determined to be important medical event. Additional information has been requested.
