Write-up: On 03/30/99 pt received first vax. On 05/06/99 pt received second dose. Approximately 12 weeks post second vax, pt returned to the physician complaining of chronic malaise and fatigue. Blood work was performed which ''ruled out'' Lyme disease, but ''Lupus'' was ''questionable'' and confirmed through additional blood work (nonspecific). The vaccinee was treated with anti-inflammatory medication (nonspecific). The pt was referred to a nephrologist. This case is considered to be medically serious. Additional information has been requested. On 11/5/99, pt received 2nd dose of Lymerix. The most recent information, received on 8/16/00, reports the outcome of the event as unknown.

Write-up: In May and June of 1999, pt received dose 1 and 2 of the Lyme vax. His health has steadily declined since then. He was taken to ER on 4/1/2000 for severe weakness, severe weight loss (down to 117.5 lbs from normal 140 lbs) in only 3 months time frame (Jan to Mar), unable to get out of bed w/o assistance. Admitted to hospital that evening. Numerous testing, including: a muscle bipsy, MRI''s, CT scans, Blood work-CP/CPK=400, etc. lead doctors to suspect polymyocitis. After 1 week of testing, he was transferred to a rehab hospital for PT and prednisone treatment, still losing weight down to 114 lbs. Further testing reveals no cancer. After 3 weeks, he is transferred to another hospital on May 6. Another MD has taken on his case and she questions dx and believes this may all be connected to Lyme vaccines he received. In May/June 1999 when onset of first symptoms began; severe fatigue, muscular weakness and pain, severe postnasal drip, frequent urination, sleep deprivation, weight loss of over 42 lbs within last 4 months, loss of motor functions and is now completely incapacitated. Also, he is having difficulty swallowing and is on IV for prednisone and hydration. He is still hospitalized and condition is worsening every day, down to 75 lbs, no mobility, can''t even feed himself now. 60 Day FU states he is now 113 lbs, weak, using a cane and scooter during the day. Continues to experience muscle discomfort and slight arthritic symptoms in his hands. Annual follow up states patient still has fatigue, general weakness, and weight loss attributed to faciitis.

Write-up: This is a spontaneous report from a 38 year old female who on 4/6/1999, had her 1st dose of Lymerix. On 4/6/2000, her arm hurt and latr that day, her finger joint was slightly swollen and stiff. All symptoms resolved by a few days without treatment. On 5/3/99, she received her 2nd Lymerix and eperienced pain at the injection site and a heavy sensation of her arm. On approx. 6/3/99, she experienced "swelling, joint pain and stiffness of the 3 middle fingers of her left hand". These symptoms progressed and she described feeling "stiff and sore" all over. In 8/99, she developed a low-grade fever and a small, red, prickly-heat-like rash on her back and buttocks. She also had a very stiff neck, backache and constant joint pain all over. She was seen by a physician who put her on doxycycline without improvement. She saw another physician who dx''d possible rheumatic fever and placed her on amoxicillin and aspirin but her pain persisted. She also had some chest pain and was prescribed Relafen. A rhematologist was consulted and dx''d possible polyarthritis. In 12/99 she had URI and was treated with Celebrex, prednisone (which she could not tolerate) and methotrexate. On 1/6/00, pt felt good and was without arthralgias on prednisone and Vioxx but when she stopped taking both meds, her symptoms recurred. On 1/21/00, MD''s dx was inflammatory arthralgias. This case is considered to be "Medically Serious" because the pt had to stop work due to joint pain.

Write-up: A persistent sore spot on left rib cage, after vaccine, building to a case of full blown Lyme Disease on 1/30/2000, with possible immune problems. Disabled with multiple symptoms. 60 Day FU states has not been able to work since 1/30/00, severe fatigue, knee, hip, and elbow and finger pain. Write-up: On approximately, 7/15/1999, the vaccinee began to experience flu-like symptoms. In early August, 1999, it was "obvious" to the reporter that her husband''s illness was "more serious than (the reporter and her husband) had thought." The vaccinee called his physician and was advised to have an immediate chest x-ray. When the physician compared it to the previous one of 2 years earlier, it was "obvious that something was wrong", as there was "fluid around his heart and at the lower porion of his lungs." He was hospitalized on 8/11/99 and released on 8/14/99. Cardiologist dx''d the pt as having pericarditis due to a viral infection. The vaccinee has been reportedly advised to forego the third dose of Lymerix. The most recent information, received on 5/15/2000, reports the condition of the vaccinee as unknown.
