Write-up: Patient stated she could feel the vaccine going all of the way down her arm. Her mother reported to us the next morning that patient had Swelling at the injection site that went to the top of the shoulder, fatigue, muscle/joint pain, chills and nausea.

Write-up: About 1.5 hrs after injection, patient reports she developed headache, felt her throat was closing up, and felt like she could not breathe. Used albuterol inhaler 4 times and got relief. Symptoms were subjective. EMS not called. Stable when seen by mom. Has hx of severe shot phobia.

Write-up: Mother refers that the patient presented pain and weakness in legs and back and problems or difficulty breathing in the afternoon post vaccination. Write-up: Soreness at site of injection - started 1 hour after injection progressed to firm nodule that was tender and warm. Resolved after 10 days. Headache on day 1 only. Soreness to neck on side of injection - resolved in two days. Fatigue - resolved in 2 days.

Write-up: Patient received TRUMENBA on his Rt upper arm. The next day he told mom his arm was slightly tender and red. He states the next day this resolved after using ice and rest.
