Write-up: In May, 1999 and June 1999, the pt received his first and second doses of Lymerix. Approximately 8 months following dose 2 on 2/1/00, the pt experienced a swollen, purple, warm, and tender knuckle, and morning stiffness in his hand. No treatment was given. Lymerix was discontinued. The most recent information received on 2/8/00, reports the condition of the pt as improving. his knuckle is less swollen, less tender, less purple, and less warm.

Write-up: On 4/5/99, the pt received his first IM left arm dose of Lymerix. Immediately following vax, the pt experienced soreness at the injection site which resolved. On 5/13/99, the pt received th second dose of Lymerix, immediately he experienced bilateral shoulder and elbow pair which was dx as rotator cuff tendonitis left shoulder. Medical record notes: 8/20/99, the pt experienced weakness, fatigue and abdominal distension without pain or nausea. Exam: +2, tenderness over the right lateral epicondyle, although ROM is good, no particular signs in the right shoulder. Assessment: weight gain, weakness and abdominal distention, and tendonitis. The MD advised ice to the area. On 10/5/99, the pt denied any injury to the shoulder or elbows. He did admit to a fair amount of lifting activity at his job previously, but nothing out of the ordinary more recently. Pt has palpable tenderness over the posterior aspect of the shoulder on the left. He had painful internal and external rotation with mild restriction of both. He had mild tenderness over the Lateral epicondyle greater than medial epicondyle, left greater than right. Neurovascularly intact. Assessment of hypertension and rotator cuff tendonitis. Treatment increase Norvasc and stretching exercises to perform. On 10/27/99, with Daypro, pain is a little better. His Mobility has also improved. However he continued to have symptoms. Left shoulder has mild tenderness in the biceps tenson insersion as well as the posterior scapular ridge area. Has full ROM although with discomfort at the extremes. On 12/21/99, the pt finished a brief course of physical therapy without a whole lot of changes in his left shoulder. The pt did not feel that the Daypro added a whole lot either. Pain primarily with pressure or sudden abduction. Internal and external rotation just slightly limited. The most recent information received on 3/17/00, reports the condition of the pt as unknown.

Write-up: On 1/4/00, the pt received the first dose of Lymerix. The following day, she experienced pain in her shoulder and pain in the front of her legs in the muscles. She could not move the injection arm and has lost movement in that arm. She experienced fatigue and has had headaches which have lasted eight weeks so far. She was seen by an MD on 2/14/00 and it taking Motrin. The pain is worse at night, Motrin does not help the pain. The MD''s notes state that the pt still experienced pain in the outer upper left arm with certain motions, for example reaching for the car shoulder strap. She occasionally got pain in the thighs or in shoulder. She has also experienced gastrointestinal upset when taking Motrin and stopped taking it. Pain with movement of shoulder was experienced. Diagnosis is reactive arthritis, (very like from Lymerix), serum sickness and some prolonged soreness at the injection site. The most recent information received on 3/15/00 reports the condition of the pt as ongoing. The headache persists and she has lost movement in her injection arm but all other symptoms have resolved.

Write-up: 05/04/00, one hour post vax, pt became nauseated, chilled, felt like flu. Arm became red and sore for 2 - 2 1/2 hours following injection and lasted several days.

Write-up: Subsequently post vax, the vaccinee experienced a severe itchy maculopapular rash all over his body. Additionally, he experienced a mild headache, and five episodes of abdominal pain which passed each time with a bowel movement. The vaccinee''s symptoms were treated with Medrol Dosepak and Zyrtec.
