Write-up: This spontaneous report as received from a nurse refers to a patient with unknown age and gender. For many years, since high school, the patient had suffered with constipation and abdominal "issues", which she had gone to many doctors for (2 primary care docs and 2 GI (gastrointestinal) specialists, and had a colonoscopy), to no avail. It was reported that the patient had been in a old study that''s been closed for about 10 years and was vaccinated with GARDASIL (dose, route , lot # were not reported) or placebo on an unspecified date. The patient discovered she had Hashimoto''s in March 2013. She was diagnosed with hypothyroidism at age 23 and have been on SYNTHROID. In the last couple years these GI problems had become more uncomfortable, including heartburn, nausea, and gas after eating anything. These symptoms interfered with the patient''s well-being, sex life/relationship, travel, daily activities, social life, etc. Since conventional medicine and approaches have not helped with the patient''s stomach, the patient went to see a Naturopath. The Naturopath advised the patient went on the GAPS diet, which was a ketogenic diet. The patient had been on this diet for about 6 week. A lot of her GI problems have improved, which was good. But, the patient felt like crap from not getting enough carbohydrate. The patient had chronic headaches, weakness. The Naturopath says this was due to a "die-off reaction" from the "bad bacteria" dying. The Naturopath upped the dose of SYNTHROID once discovering the patient had Hashimoto''s and that the patient was still hypothyroid. She took the patient from the 75 mcg the patient had been on for years all the way up to 125 mcg on approximately 19-MAR-2013. After 6 weeks on this higher dose, the patient had blood drawn Tuesday, on 30-APR-2013, and was now hyperthyroid, with a TSH (thyroid stimulating hormone) of 0.5. The patient had been having a lot of anxiety the past 2 weeks, from approximately 19-APR-2013 and she thought she was going nuts. At the time of reporting, the patient''s outcome was unknown. This is one of several cases with the same reporter. Additional information has been requested.

Write-up: Admitted 6/13 with headache, weakness and progressive mental status changes. CSF with 120 WBC, culture negative, MRI consistent with acute disseminated encephalomyelitis. Treated with IVIG and steroids. Became comatose, required intubation. Now recovering.

Write-up: After receiving HPV vaccine patient had a near syncopal episode with altered mental status lasting almost 45 minutes. After 45 minutes pt had returned to pre-vaccine status and was observed in office.

Write-up: She became very nervous right after the shot. Seemed confused. Had trouble getting her thoughts together. She became very depressed, started her period (which she takes medication to stop her period, having a period is very unusual), she ran a fever, loose bowel movements. Ever since the shot she has been very depressed and confused. She was doing very well prior to the shot. She has a brain injury and had been sick for sometime due to an emotional breakdown. Had been on medication to help calm her and help her to not be confused anymore. She had been doing so well we were able to wean her off of the medication. Now she is back on it cause once she got the Gardasil shot she changed right away. has even told us the shot made her sick. She has special needs and for her to make a connection to the shot and her not feeling well, that says a lot.

Write-up: This spontaneous report as received from a consumer refers to a 16 year old female patient with learning disabilities with maths and science and Drug allergies/reactions to amoxicillin (manufacturer unknown). Approximately in 2012 (reported as two years ago) the patient was vaccinated with 3 doses of GARDASIL. On 31-JAN-2014 the consumer reported that her daughter had changed. On 31-JAN-2014 the patient started to become dizzy, had convulsions and pseudo seizures. For these adverse reactions the patient had to go two hospital (3 times total) and had to stay there for a couple of days. Approximately in March 2014 (reported as couple weeks ago) the patient experienced mental change. The patient started to become "upset, lashing out at people, repeating herself, having high anxiety, wanting to speak constantly, verbally abusive, having suicidal thoughts, wants to slit her throat and stuck her fingers down her throat to make herself throw up". The patient also started to slur her speech asking her mother if she had a "stroke" and wanted to know if she had "cancer". The patient was admitted to the hospital and then was transferred to another hospital. The reporter stated that the patient was getting beat up by the other patients, but it might be (the patient) provoking them. The outcome was reported as not recovered/not resolved. The reporter believes that GARDASIL had caused the events in her daughter. Additional information has been requested.

Write-up: 18 y/o male with no previous PMHx who presents with new onset seizure and altered mental status. Symptoms initially began 4 days prior to admission after he had been soldering with lead for 4-5 hours at work with no mask. Headache per mother was localized to the posterior occiput and he tried to take some Aleve with no relief. Two days prior to admission he developed left sided neck pain that was not associated with neck stiffness. His mother also noted that he was having some episodes of chills, but no fever. One day prior to admission he developed lower back pain in addition to the previous symptoms and presented to his PMD who examined him, ordered blood work and felt he had a muscle strain. He was given the HPV vaccine at that time. The morning of admission at 10:30am his mother heard noises coming from the patient''s bedroom and went to check on him. She found him having seizure activity with vertical nystagmus, tonic-clonic movements of the upper extremities and was not responsive. There was no cyanosis or incontinence. She denied any facial or limb twitching. The mother reports she may have felt that the left upper extremity started shaking first before the rest of his body but she is not sure. It lasted for 2-3 minutes before stopping spontaneously. ROS: No photophobia, no phonophobia, no URI symptoms, no recent travel, no fever, (+)co-worker who recently traveled to vietnam, (+) decreased appetite.
