Write-up: Information has been received from a doctor of psychology concerning an unknown number of patients of unknown age and gender. The patient''s relevant medical history, past drug therapy and concomitant medications were unknown. On an unknown dates, were vaccinated with unknown doses of GARDASIL with frequencies and for indications unknown (route, dose and lot number not reported). On an unknown date, the doctor of psychology reported that she heard a report on the news on 13-SEP-2011 in which the speaker mentioned that GARDASIL caused mental retardation. It was unknown if the patients sought medical attention and if there was any laboratory data. At the time of the report it was unknown if these patients continued on GARDASIL and the statuses of their mental retardation. This is one of several reports received from the same source. Upon internal review, mental retardation was determined to be an other important medical event. Attempts are being made to obtain additional identifying information to distinguish the individual patients mentioned in this report. Additional information will be provided if available. This is a hearsay report in the absence of an identifiable patient. Attempts are being made to verify the existence of a patient.

Write-up: Information has been received from a physician concerning an 18 year old female who on 13-OCT-2010 was vaccinated SC with the first 0.5ml dose of GARDASIL (Lot# 0938AA/1354AA, exp date 26-JUN-2014) and on an unspecified date was vaccinated with the second dose of GARDASIL (Lot# and route not reported). On 15-NOV-2011 the patient experienced weakness in her body and lower extremities, nausea, headaches, vomiting and an altered mental status after receiving a second injection on GARDASIL. The patient was hospitalized in the ER at an unspecified location for an unspecified duration of time. MORPHINA (strength and manufacturer unspecified) was given for her headaches. The doses of GARDASIL were also given outside the recommended dosing schedule for this product. The patient did many unspecified tests. At the time of the report, the patient was not recovered from the events. Additional information has been requested.

Write-up: Information has been received from a physician concerning a female patient with allergy to other unspecified vaccine and a history of becoming disoriented and loopy when other vaccines administered who on an unspecified date was vaccinated with the first 0.5 mL dose of GARDASIL. Subsequently the patient experienced becoming disoriented, lethargic and loopy. She would not receive any further treatment following the first dose. After vaccination the adverse event improved. There was no treatment given for the adverse events. The vaccination of GARDASIL was discontinued and not reintroduced. The patient did not seek medical attention. At the time of the report, the patient recovered from the events. Follow up information has been received from the physician concerning the 18 years old student patient with no illness at the time of vaccination and no pre-existing allergies, birth defects and medical conditions who on 24-APR-2012 at 15:02, was vaccinated with first dose of GARDASIL (lot # 672652/0022AE) IM in the left arm. It was reported that on 25-APR-2012, the patient felt hot, felt emotionally unstable, unstoppable crying, patient felt "off today" and had trouble focusing. Parents stated that she was speaking slowly and was not herself. On an unspecified date, flu test showed negative result and blood pressure was normal. On 26-APR-2012, the patient recovered. The patient required and ER visit. No further information is available.

Write-up: Very confused, out of it, brain wasn''t working, couldn''t think, pain of injection site, eyes swollen, very bad headache, very dizzy, nausea 7 AM - trouble breathing, very difficult to breathe, very dizzy, very confused, brain fuzzy/not working right, shortness of breath, leg aching, bad stomach ache, body hurt. That lasted all day and into the next day. Very, very worried and scared patient!! Doctor on call said to take Zyrtec and 2 Benadryl and see if that helped. Patient was very sleepy but still had symptoms... got a bit better, then worse with confusion. Very confused at times. Still on 3-25 - still confused and brain not clear.
