Write-up: Intussusception. Pt had blood in stool & vomiting at 3AM. Pt came to office with blood in diaper after stool. Pt sent to hospital. Air contrast enema was given for reduction which was successful. 1/12/09 Reviewed hospital medical records of 11/11-11/12/2008. FINAL DX: Intussusception Records reveal patient experienced blood ini stool. Developed reduced oral intake & vomiting. Admitted overnight for observation s/p enema reduction of IS. Progressed well & d/c to home.

Diagnostic Lab Data: GUAIAC POSITIVE IN OFFICE 12/15-08-records received US abdomen 12/10/08-dilatated fluid filled bowel loops with small anount of free fluid. Intussusception identified under fluoroscopy. Air contrast enema revealed ileo-colic intussusception to transverse colon partially reduced with air reduction enema. Surgical intervention required. 1/14/09-records received-US normal. Air enema unsuccessful reduction.

Write-up: VOMITING AND BLOODY DIARRHEA FOR 1 DAY. FEVER 100R IN THE AM. SEEN IN OFFICE, EVALUATED WENT HOME AND C/B ABOUT BLOODY DIAPER. RETURNED TO OFFICE. SEEN BY DR, EVALUATED WHO THEN CONTACTED SURGERY AT MEDICAL CENTER AND TRANPORTED BY AMBULANCE. PT UNDER WENT SURGERY FOR INTUSSUSCEPTION 12102008. 1/14/09-DC Summary received for DOS 12/10/08-DX: Intussusception. Presented with abdominal pain, and bloody stool. Surgery for manual reduction. Developed post operative ileus. Resolved and discharged. Follow-up: My nurse manager filled out a VAERS report online on 12/11/08. The temporary VAERS E-Report number is E-27156. I just wanted to emphasize this was a healthy four month old infant. He rec''d his second rotavirus vaccine, ROTATEQ on 12/5/08. On evening of 12/9/08 he developed vomiting, most likely this was due to obstruction from intussusception. On 12/10/08 he presented to our office in the am, same NP and felt be have Gastroenteritis. Later that day he had a bloody stool, came back, diaper was grossly bloody, was irritable and intermittently lethargic. Went to tertiary care center via ambulance. He required emergency surgery that night for intussusception. So, he developed symptoms of intussusception within four days of receiving ROTATEQ vaccine.

Write-up: Vomiting starting on 12/2/08 - led to hospitalization. Final diagnosis was intussusception. 01/02/08 MR received for DOS 12/4-5/2008 with D/C DX: Intussusception. Pt presented with 2 day hx of emesis, loose stool most recent was black in color and and inability to tolerate po feeds. Upper GI (+) for IS which was reduced by barium enema. D/C next day when tolerating po feeds well.

Write-up: Pt had acute onset of intestinal ileus after a procedure of 3-4 days of nausea and diarrhea, becoming frank rectal bleeding. Pt diagnosed with intussusception requiring a 6 inch resection at caecum and small bowel. 1/23/09 Reviewed hospital medical records of 09/17-09/23/2008. FINAL DX: Ileocolic & colocolonic intussusception; colectomy w/resection of terminal ileum & primary ileocolonic anastomosis. Records reveal patient experienced lethargy, vomiting, dehydration, abdominal distention x 24 hr. Air enema resulted in bowel perforation & pneumoperitoneum. To OR & PICU. Tx w/TPN. Diet advanced, tol well & d/c to home. 1/12/09 Reviewed hospital medical records of 9/14-9/17/2008. FINAL DX: acute ileus, possible IS vs volvulus; dehydration, resolved; viral enteritis; bilsteral supprative OM. Records reveal patient experienced bilateral supprative OM, vomiting, lethargy, red cheeks, low grade fever . Developed bloody stool. Transferred to higher level of care. 1/14/09 Reviewed PCP medical records of 8/7-12/29/2008. Records reveal patient experienced regurgitation, gas, arching back, crying during feeding on 8/7/08. Formula changed to soy. RTC 8/12 w/Acute URI & rhinnitis. RTC 10/1/08 doing well post op.
