Write-up: Pt. rec. vaccine on 02/01/16, tolerated well. The following morning she awoke with severe pain, redness and puffiness to (L) arm. Redness and pain with swelling continued and pt. returned to the office on 02/03/16 with concerns of adverse rxn warmth, redness and swelling documented throughout entire Lt upper arm. Pt advised to do BENADRYL, MOTRIL for 48 hours.

Write-up: Left deltoid area. Local reaction around injection - redness - not confluent - gave 25 mg BENADRYL -$g and resolving in 15 min - down to 2 inches.

Current Illness: Yes. Sore throat, headache, congestion, ?cough on 1/22 which developed into bilateral cervical lymphadenopathy, fever, and exudative pharyngitis --$g Group A strep positive on throat PCR on 1/25/2016. Diffuse headache developed on 1/27 and responded to Tylenol; recurred on 1/28 and went to see her primary care provider. Received vaccines at that appointment.
Preexisting Conditions: Asplenic (history of hereditary spherocytosis, underwent splenectomy 7/2014). Was not on penicillin prophylaxis (family discontinued).

Diagnostic Lab Data: Still having profound fatigue. EBV serology: 5/9/14 (neg IgM / neg IgG / neg EBNA); 5/15/2014 (pos IgM / neg IgG / pos EBNA); 1/25/2016 (neg IgM / pos IgG / pos EBNA). 1/25: group A Streptococcus throat probe positive. 1/28: CSF (on Augmentin) o 111 WBCs (diff 92% lymph, 4% PMNs, 4% monos), 27 RBCs, glucose 69, protein 70 o viral culture negative for cytomegalovirus (CMV) o HSV and Enterovirus PCRs negative o Gram stain few WBCs, no organisms; culture negative 1/28 blood culture (pre-antibiotics) negative. 1/29 UA negative 1/29 mycoplasma IgM negative. 1/29 specimens to Department of Public Health / VRDL o Nasal swab negative for coronavirus (4 strains), hMPV, parainfluenza types 1-4, RSV, enterovirus, rhinovirus o Throat swab negative for coronavirus (4 strains), hMPV, parainfluenza types 1-4, RSV, enterovirus, rhinovirus o CSF, bacterial PCRs negative, enterovirus PCR negative 1/30 Mycoplasma IgM/IgG negative. 1/30 Mycoplasma throat PCR negative. 1/30 Enterovirus PCR (stool) negative. 1/30 Enterovirus PCR (throat) negative. 1/30 CMV PCR negative. 1/30 EBV PCR negative. 1/30 Bartonella IgM/IgG negative. 1/30 resp viral PCR panel negative Imaging 1/29 EKG normal 1/28 CT head / non-contrast: no abnormality, limited exam
CDC Split Type:

Write-up: From ED notes: sudden onset decreased appetite, malaise, tense, anxious, clammy, slightly dissociated; and with visual hallucinations/metamorphosia (objects seemed to be very far away). Denies any auditory hallucinations. She started having unusual episodes lasting 1-2 minutes, described as starting with tingling in her hand followed by muscle tightness (everywhere), dizziness, blurred vision, sometimes double vision. Patient and family estimate that she had $g20 such episodes on 1/28. In between, she seemed a bit slow/groggy slurred speech, but was oriented and not confused. She was staring and had a "far-off look" at times. In the ED she had an episode of head tremor "like Parkinson''s".
