Write-up: This case was reported by a physician via other manufacturer and described the occurrence of injection site swelling in a female patient who received BEXSEROL. Co-suspect products included BEXSERO. On an unknown date, the patient received the 1st dose of BVEXSERO .5 ml and the 2nd dose of BEXSERO .5 ml. On an unknown date, an unknown time after receiving BEXSERO and not applicable after receiving BEXSERO, the patient experienced injection site swelling, injection site erythema, injection site pain, injection site swelling, injection site erythema and injection site pain. On an unknown date, the outcome of the injection site swelling, injection site erythema and injection site pain were recovered/resolved and the outcome of the injection site swelling, injection site erythema and injection site pain were not recovered/not resolved. It was unknown if the reporter considered the injection site swelling, injection site erythema and injection site pain to be related to BEXSERO. It was unknown if the reporter considered the injection site swelling, injection site erythema and injection site pain to be related to BEXSERO. Additional details were provided as follows: This case was related via Merck National Service Center. This case was reported by a physician, who herself was the patient''s mother. The reporter stated that the injection site reactions were worse with the 2nd dose of BEXSERO than with the 1st dose. The reporting physician agreed to follow up for information about the patient''s injection site adverse events after the vaccination with BEXSERO. No additional information was provided. This case has been linked to case US2016GSK015758, reported by the same reporter.

Write-up: Left deltoid/upper arm with redness and tenderness about the size of a softball. No fever and no other treatment ordered. Dad calling and stating child got HPV and Hep A vaccines on Monday and his arm is swollen now. Dad wondering if they should be worried. Dad calls to report pt got vaccines 2 days ago. The area he got the shots is now red and warm to touch. The red area is about the size of a softball per father. Child is not able to raise arm normally. Appt recommended and dad agrees. Child is at school and father is hoping for something after school. Appt made for 4pm today with Dr. Father will call back before apt time if area worsens at all.

Write-up: Patient c/o nausea after breakfast. Attempted to follow normal routine for school. 11AM he c/o increased nausea and not feeling well. T=102.8. Placed on Medical Isolation to rest in room. Placed on clear liquid diet and. Admin Tylenol 650mg Placed on Vital Signs and Tylenol 650mg Q 4 hours and encourage PO fluid intake. Monitored. 2/13/16 7AM, he was afebrile and no c/o voiced. VSS. Returned to normal routine.

Write-up: This case was reported by a consumer and described the occurrence of vaccination failure in a male patient who received BEXSERO. Co-suspect products included BEXSERO. Concurrent medical conditions included exposure to communicable disease (total 3 students including the patient had become sick with meningococcal disease). On an unknown date, the patient received the 1st dose of BEXSERO. In October 2015, the patient received the 2nd dose of BEXSERO. In February 2016, an unknown time after receiving BEXSERO and 91 days after receiving BEXSERO, the patient experienced vaccination failure (serious criteria GSK medically significant), bacterial meningitis (serious criteria hospitalization and GSK medically significant), headache, stiff neck and fever. On an unknown date, the outcome of the vaccination failure was unknown and the outcome of the bacterial meningitis, headache, stiff neck and fever were recovered/resolved. It was unknown if the reporter considered the vaccination failure, bacterial meningitis, headache, stiff neck and fever to be related to BEXSERO and BEXSERO. Additional details were provided as follows: The patient was previously healthy and had no chronic conditions or medications. There were total 3 students (including the patient) who had become sick with meningococcal disease outbreak at the university. Approximately 3 and half months after vaccination with 2nd dose of BEXSERO, the patient experienced the events. The patient was hospitalized for 3 days and was released. There were no complications. The reporter did not know about any results of diagnostic testing on cerebrospinal fluid. This case was considered as suspected vaccination failure since the details regarding confirmatory laboratory tests were not provided.

Write-up: Vaccine administered, pt tolerated well. The following morning pt awoke with pain at injection site. Unable to lift arm due to pain. Pain continued for the next few days as well as redness, swelling and warmth at injection site. Pain was severe and lasted app. 3 days. Redness, swelling and warmth lasted 3-5 days. Pt was prescribed MOTRIN.
