Write-up: Case number PHEH2015US006239 is an initial spontaneous report from a nurse received on 30 Mar 2015. This report refers to a 18 year old female patient. She had her normal menstrual period on 18 Feb 2015. She was vaccinated with BEXSERO (batch number: 146101) intramuscularly on 24 Feb 2015. On the same date, after vaccination she experienced sore arm. On 30 Mar 2015, the nurse reported that the patient was currently pregnant and possibly 6 weeks pregnant (not confirmed). The patient did not believe that she was pregnant at the time of vaccination. The patient did not receive the second dose of BEXSERO at the time of reporting. The outcome, seriousness and causality of the event sore arm was not reported. This case was identified with a wrong pregnancy outcome during checks: This prospective pregnancy case was ongoing.

Write-up: STARTED WITH FEVER, SORE THROAT AND MUSCLE ACHES. THEN DEVELOPED SWELLING OF LIPS AND ARM. GIVEN BENADRYL BUT SWELLING INCREASED AND INCLUDED THE THROAT. TAKEN TO ER WHERE ONE DOSE OF ORAL STEROIDS GIVEN. ALREADY IMPROVING FROM PREVIOUS DOSE OF BENADRLYL. INSTRUCTED TO CONTINUE BENADRYL FOR 2 MORE DAYS. IF BENADRYL DOESN''T HELP WILL CONTINUE STEROIDS.

Write-up: 01/13/16 after injection pt reported feeling nauseous and very warm. Pt instructed to lie down - gave pretzel and H20. Pt left office feeling back to normal within 1/2 hr. Pt returned to office 2 days later for PPD read. Injection site of TRUMENBA erythematous, tender. Pt reports pain and difficulty raising her arm for the 24 hours post injection.

Write-up: Patient complained of nausea after receiving GARDASIL vaccine. She complained of blurry vision, back aches, tingling sensation in body and throat pain. Patient given oxygen via face mask. Patient vital signs stable alert and no loss of consciousness patient monitored on exam table and alert and oriented, blood pressure, HR and oxygen stable and sent for EKG.
