Write-up: She had mild migraines lasting a couple of days for the last year and a half. The next day after She received the vaccine she started a severe migaine headache that has not stopped for more than a couple of times for two days, one time after being hospitalized and receiving solumedral and DHE. Went to the ER three times to try to relieve the headache. On preventative medication for her daily headache but it has not helped yet. Has been out of school since 4/18.

Diagnostic Lab Data: Came in the next day with myalgias, fever 99.3, nasal congestion; lasted 24-48 hours.

Write-up: Immediately following administration of second vaccine, patient became nauseated, weak, color paled, assisted to exam table; laid down, given sips of soda and was feeling better within 5-10 minutes after sitting up and feeling better approximately 10 minutes. Home with mom. Returned next day with complaints of fever, headache, and weakness. Advised fluids, rest and monitor.

Write-up: Patient received MMR and Menactra 5/10/05. Then seen 5/12/05, 24 hours later developed high fever 104. Seen 5/12/05 P/E were normal. Febrile illness. Resolved in 36 hours. Follow up on 7/19/05: Also complained of new onset headache associated with AE.

Diagnostic Lab Data: CBC, Lumbar puncture, blood and urine culture, drug screen, Ct scan brain were done. Cultures and PCR (CSF) negative. EEG NL.

Write-up: 5/6/05 HBV and Menactra were administered. 5/11/05 bronchitis, treated with Biaxin/Albuterol and Prednisone; 5/14/05 Diarrhea, vomiting; 5/15/05 generalized seizure, ER, Dx: viral meningitis. Pt 15 yo child was seen in our office on 5/6/05 for well visit. He was given HBV and Menactra. No calls received from parents regarding any side effects. On 5/11/05 child was seen for cough, wheezing. He was diagnosed bronchitis/RAD, started on Biaxin XC, Albuterol Inhaler and Prednisone for 5 days. As per mom, he developed vomiting and diarrhea on 5/14/05. On 5/15/05 he developed generalized seizure. No h/o high fever or stiff neck or rash. Child was taken to nearest hospital by ambulance. CBC, Lumbar puncture, blood and urine culture, drug screen, Ct scan brain were done. Child was started on Ceftriaxone, Dexamethasone, Vancomycin and transferred to another hospital. He was admitted because of change in mental status. He was also started on Acyclovir. EEG NL, Neuro was consulted, child was discharged on no medication. All antibiotics were discontinued because cultures and PCR (CSF) negative in deciding for herpes simplex. the final diagnosis was viral meningitis and seizure. Please see enclosed lab results.

Write-up: Child presented for 11 year PE on 5/21/05. Menactra administered IM right deltoid. Td administered IM left deltoid. Onset fever Saturday PM; increased to 103.6 oral. No other symptoms. Patient seen in office on 5/23/05. Exam NL with 2" red, warm area right bicep; tender to palp.

Write-up: Intermittent Dizziness started within 24 hours. has continued for 6 days. Dizziness initially lasted several hours regardless of position (prone, seated, standing) and transition to different positions. By day 4-5, it was 3-4 hours and worse in the morning. By day 6, it is getting less severe.
