Write-up: Meningococcal Menactra given at 11 am Aug 9 2005. That evening patient had chills and couldn''t sleep most of night because of them. Woke up Aug 10 with headache which persisted until the evening. Intermittent chills and slept most of afternoon. Left side neck slight stiff. All symptoms dissipated/gone by morning of Aug 11.

Write-up: 2 hrs after vaccine head felt swimming lied down to nap. 5-6 hours after vaccine fever noted and joint stiffness. 12-36 hours after vaccine continued head felling swimmy still. Frequent loose Bowel movements noted (about b/d). Fever and chills that evening (Tues). Wed continued head symptoms, swelling in neck noted , slight swelling of face. Continued frequent Bowel movements.

Write-up: 8/16/05 - 16 hr post vaccine developed fever, temp 101, headache, dizziness. MD contacted and recommended Advil 800 mg q. 8 hr & to push fluids which the client stated he did. Symptoms continued with shoulder and lower back ache finally subsiding that same evening.

Write-up: 24 hrs after receiving Menactra vacc left deltoid, pt experienced local redness at injection site, then felt numbness left upper and lower extremity and acute confusional state lasting 4-6 hrs. Pt also began vomiting and had transient (<30min) signs Bells Palsy. Recovered on 6 hours but brief recurrence all above symptoms 7 days later (lasted <1hr). Meds given Benadryl and Reglan. Exam on 8/18 entirely normal except very mild left facial weakness.

Write-up: Petechiae noted on lower extremity on am of 8/7/5. Had no other URI, N/V or any other symptoms preceding this occurence. No fever.
