Admitted 11/16-11/17/2007. Enema successfully reduced IS. Admitted for overnight observation. Did well & was d/c to home. Readmitted 11/19-11/20/2007 for recurrence of vomiting & bloody stools. 2nd abdominal US (+) for IS & was successfully reduced w/air contrast enema. Kept overnight for observation. Write-up: Abdominal pain, R/O intussusception - URI, cold and cough 1 week. Intussusception continued U/S. + Abd X-ray. Successful reduction by Barium enema. On Albuterol too, ? RAD. Received flu vaccine 10/30/2007, SP lot # U2462AA, LAT. 12/20/07 Patient experienced runny nose, cough/cold symptoms x 1 week w/diarrhea x 3 days & vomiting x 1 day. When seen in clinic, parent revealed patient had been in hospital 11/10-11/11/2007 for IS. 12/28/07 Reviewed hospital medical records which reveal patient experienced cough x 2 wks,irritability, colicky abdominal pain & vomiting x 1 day. Family member had been ill w/gastritis 1 wk prior. Admitted 11/10-11/11/2007. Surgery consult done. Progressed well s/p reduction & d/c to home. FINAL DX: intussusception, enema reduced.

Write-up: Information has been received from a physician concerning a female who began the vaccination series at "2 months of age" (date not reported) with Rotateq. It was reported that one month after receiving the third dose of the vaccine (date not reported) she developed intussusception and was hospitalized. Diagnostics were not reported. The intussusception was treated with a barium enema. At the time of this report, it was reported that the patient had recovered. Intussusception was considered to be life threatening and an other important medical event by the reporter. Additional information has been requested.

Write-up: Information has been received from a nurse concerning an approximately 13 week old male who on 25-OCT-2007 was vaccinated PO with his first dose of Rotateq (lot# 656379/0505U). Concomitant vaccinations administered on that same day included a dose of DTaP, a dose of PedvaxHIB, a dose of Prevnar. On approximately 22-NOV-2007 the patient experienced intussusception, was hospitalized and underwent surgery on 22-NOV-2007 (length of stay and type of surgery not specified). Subsequently, the patient recovered. It was noted that the patient discontinued the series for the second and third doses of Rotateq. There was no product quality complaint. Intussusception was considered to be an other important medical event (surgery) by the reporter. Additional information has been requested. 01/10/2008 Vax info updated in database. 02/12/2008 MR received for DOS 11/22-23/2007 with D/C DX: Intussusception. Infant presented with vomiting, bloody, brownish stools, refusal to eat and screaming. Several day hx of nasal congestion. Pt underwent reduction of IS via enema in ER. Admitted for observation post procedure and D/Cd next day.

Write-up: 4 days after vaccines, patient developed bilious emesis and rectal bleeding. Abdominal ultrasound at hospital showed intussusception - non reducible by air enema. Patient underwent exploratory laparotomy for reduction of intussusception and resection of ileocecal enteric duplication cyst found at that time. 2/5/08-records received for DOS 12/17-12/19/07-DC DX: Intussusception. presented to ED with C/O vomiting greenish yellow 6 episodes. BRBRPR, no stool. Abdominal pain, increased sleepiness, decreased wet diapers. Exploratory laparotomy, resection of ileocecal enteric duplication cyst. appendectomy.

It appears that the reported lot # was for the 9/18/07 dose not the 11/13/07 dose. 5/30/2008 MR received for DOS 1/2-3/2008 with DX: Intussusception. Infant presented to local hospital with belly pain, vomiting and bloody diarrhea. Pt had been feeding poorly and had fewer wet diapers. Reduced by Barium enema and admitted for o/n obs.

Write-up: Pt began vomiting 12/18/07 PM to ER 12/19/07. Dx with intussuseption. A barium enema was attempted, but did not reduce. Surgical bowel reduction was successful. An appendectomy was also performed. 02/01/2008 MR received for DOS 12/19-21/2007 with DX: Intussusception. Pt presented with 1 day hx of vomiting (16 episodes). Had bloody stool in ER. Attempted to reduce with Barium enema but unsuccessful. Pt had exploratory lap with reduction of ileocolic intussusception, appendectomy, and biopsy of mesenteric lymph nodes. Mesenteric lymph nodes noted to be enlarged in OR. D/C on POD #2

Write-up: Information has been received from a physician concerning a 6 month old male who on 27-DEC-2007 was vaccinated with the third dose of Rotateq. On 04-JAN-2008 the patient experienced intussusception and was hospitalized for an unspecified period of time. The patient had a "contrast enema" performed on an unknown date, results not reported. Subsequently, the patient recovered from intussusception. Upon internal review, intussusception is considered an other important medical event. No product quality complaint reported. Additional information has been requested. 01/22/2008 Vax record received and updated in database. 01/29/2008 MR received for DOS 01/4-5/2008 with DX: Intussusception. Child presented to ER with 1 day hx of diarrhea with screaming and abdominal pain. Reduced by Barium Enema. Pt tolerating diet and D/Cd next day.

Write-up: Infant on 11/27/07 began having small amount of diarrhea (non bloody)and vomiting. Later that evening there were no stools passed and worsening colicy abdominal pain and bilious vomiting. Discovered to have an intussuseption unreducible via air contrast barium enema. Infant taken to OR immediately and intussuseption reduced surgically. 02/05/08-records received for DOS 11/27-11/28/07-DC DX: Intussusception, status post appendectomy and laparotomy with reduction of intussusception. Presented to ED with C/O abdominal pain and vomiting. PE: bloody mucous stool.
