Write-up: Pt called in c/o sore arm and swelling and pain with difficulty moving arm. Advised to take Nsaid and increased fluids, left arm swollen shoulder to elbow, redness, no fever. 8/8/05 advised to go to urgent care, didn''t. No to taking antiinflammatory or Tylenol, swelling decreased still having difficulty moving arm, advised to seek care.

Write-up: Patient received Meningococcal Conj. vaccine. Did not wait more than couple minutes to exit office. Fainted outside of office and parent states hit head. Severe headache post fainting. Examined by physician in office right after.

Write-up: Received Menactra in our office 7/27/05, saw his allergist 7/30/05 and had to receive a breathing treatment, MD questioned if related to Menactra. 50 cent size raised warm area around injection site.
