Write-up: 3-4 days post iz, developed sore throat and tiredness. Shot contained 3-4 days and then started bouts of diarrhea and decreased appetite. No fever noted. Resolved 10-11 days later.

Write-up: 6 hours after receiving vaccines (and immediately after intense football practice on hot day) patient developed fever to 102.2, chills, and aches, symptoms lasted 2 1/2 hours, then resolved after IV fluids, and ibuprofen. No further symptoms.

Write-up: Patient received the vaccine on 7/28/2005. Has c/o flu-like symptoms approx 24 hours later: achiness and painful joints throughout her body. On 8/3/05 c/o moderate to severe, frequent to constant neck pain bilaterally. She c/o increased pain on cervical flexion, extension and right and left lateral flexion. Soto Hall''s test was positive. Her medical doctor''s office was notified.

Write-up: Approximately 2-3 hours later patient became dizzy, nausea, chest pain, abdominal pain, leg pain, joint aches, was unable to walk alone, actually fell to the floor once and patient was admitted to hospital for 24 hours for observation. There were no other significant events to attribute to this acute change in patient''s status.

Write-up: Patient came back to clinic at 1 PM 8/10/05 with father. He stated he had meningitis short at clinic today at 10:20 AM. He felt good; no problems after shot so left right after. While driving home, he felt dizzy and then did not remember anything after until he woke up and he had collapsed into a parked car on the other side of highway. Didn''t have event time of crash.
