Write-up: Pt given Rotateq vaccine PO 10/02/06. Pt diagnosed with intussusception 10/08/06 and had surgical reduction 10/09/2006. 11/16/06 Received medical records from hospital which reveal patient had history of 3 days of vomiting & 1 day of blood tinged stool prior to admission on 10/9/06. Afebrile. Air enema success only at ileocecal valve & was taken to OR for reduction of intussusception. Developed post op ileus which resolved & progressed to stable status. D/C to home on 10/12. D/C Dx: S/P intussusception, surgically resolved./ss

Write-up: Onset of vomiting and bloody stool 3-4 hours after administration of vaccines. Patient seen in emergency room and diagnosed with intussusception, reduced with air enema. 12/19/06 Received medical records from ER which reveal patient seen 11/06/06 s/p vomiting & bloody stool on day of vax. Afebrile. Lethargic. Hypoactive bowel sounds & palpable soft mass in RUQ with tenderness to palpation. Seen in ER at outlying hospital then transferred to facility which could provide air contrast enema. Kept in that ER for several hours s/p successful reduction for continued observation. Final DX: Intussusception, reduced by air enema.

Write-up: Pt visiting out of country with parents began with persistent crying on 11/25/06. On 11/26/06 awoke with diaper of blood. Diagnosed with intussusception and had partial small bowel resection performed emergency out of country. Unable to assess now. Per 60 day follow up: Currently recovered, however, he did have to have bowel resected so potentially problems later with absorption.

Write-up: Intussusception 11/5/06 - 11/09/06 reduced with Ba enema. 12/8/06 Received medical records from hospital which reveal patient experienced a week of vomiting prior to admit. No fever & no sick contacts. Lethargic on admission 11/3/06 & anterior fontanelle was depressed & mucous membranes dry. Abdomen was soft with no masses or distention. Surgical consult done. Air enema relieved the IS. No further vomiting & patient d/c to home on 11/8/06.

Write-up: On 11/29/06 pt was listless, fed poorly and had 3 bloody mucousy stools. In my office the patient was pale and listless. The patient was refered to ER. An air contrast BE was diagnostic and therapeutic for the pt intussisption. The pt spent one night in hospital and is doing well.

Write-up: Immunizations on 10/02/2006 symptoms of bloody stools 12/04 surgical reduction of intussusception 12/04. 1/22/07 Received medical records from hospital which reveal patient admitted 12/4/-12/7/06 after having had bright red bloody diaper & was fussy prior to being seen in ER on 12/4/06. GYN & surgical consults done to r/o vaginal bleeding. US showed IS. Air enema attempt to reduce intussusception was unsuccessful & was taken to OR whre it was reduced w/o difficulty. Continued to improve & was d/c to home w/o complications. FINAL DX: Intussusception, surgically reduced.

Write-up: 2 days after Rotateq, patient began vomiting 11/29/06 ultrasound on 12/1/06 showed signs of obstruction and . Abdomen-surgery on 12/1/06 revealed intussusception with gangrenous ascending and transverse colon-after, colectomy of involved segment Andre-anastomosis. Patient is doing well.

Write-up: Information has been received from a physician concerning a 6 month old male, with no medical history and no allergies, who was vaccinated with a first and second oral dose of rotavirus vaccine on 6/20/06 and 9/5/06, respectively. There was no concomitant medication. On 11/6/06, the pt was found to have blood in the stool. An abdominal ultrasound showed a small bowel intussusception. A barium enema was done after the ultrasound that showed no intussusception. The small bowel intussusception was found to be transient and reduced spontaneously. there was no blood in the stool on 11/13/06 and the pt was considered to be recovered by the reporter. Unspecified medical attention was sought. No product quality complaint was involved. No other information was provided. Upon internal review, intussusception was considered to be an other important medical event (OMIC). Additional information has been requested.

Write-up: Information has been received from a physician concerning a 23 week old male, with no significant medical history, who, on 16-Jun-2006 was vaccinated with an oral, 2.0 ml, first dose of rotavirus G1 G2 G3 G4 P1 reassortant vaccine live (human-bovine) (Lot#653661/0146F). Concomitant vaccination administered on 16-Jun-2006 included a dose of diphtheria toxoid (+) pertussis acellular vaccine (unspecified) (+) tetanus toxoid, a dose of Hib conj vaccine (tetanus toxoid) (ACTHIB), and a dose if hepatitis B virus vaccine rHBsAg (yeast). On 14-Jul-2006 the patient was vaccinated with a second 2.0 ml oral dose of rotavirus G1 G2 G3 G4 P1 reassortant vaccine live (human-bovine). There were no concomitant vaccinations given on 14-Jul-2006. In July 2006 the patient was noted to have fallen out of a shopping cart. On 02-Aug-2006 the patient developed a fever. The patient was taken to the emergency room where an x-ray and labs (not further specified) were normal. No formal diagnosis was made and the patient was simply thought to have a virus. On 17-Aug-2006 the patient was vaccinated with a third 2.0 ml oral dose of rotavirus G1 G2 G3 G4 P1 reassortant vaccine live (human-bovine). Concomitant vaccinations administered on 17-Aug-2006 included a dose of diphtheria toxoid (+) pertussis acellular vaccine (unspecified) (+) tetanus toxoid, a dose of Hib conj vaccine (tetanus toxoid) (ACTHIB), a dose of pneumococcal 4 6B 9V 14 18C 19F 23F conj vaccine (CRM197) (PREVNAR) and a dose of poliovirus vaccine inactivated (IPOL). On 28-Sep-2006 the patient developed vomiting and irritability. The patient was subsequently hospitalized due to persistent vomiting. While hospitalized intussusception was diagnosed and reduced via Barium enema. The patient recovered within 24 hours and was discharged to home. At the time of the report the patient was noted to be doing fine by the physician. The physician indicated that he would provide additional information, including medical records if it became available. No product quality complaint was involved. No other information was provided.
