Write-up: 1 day after receiving Menactra, developed increasing pain and swelling at vaccine site. (+) dizziness 3 days later x 1 day. No fever. Seen in office 4 days after immunization with 20cm induration, tender and no erythema. Treated with Keflex.

Write-up: Severe muscle pain and achiness, chills, temperature of 99.5F. Awakened from sleep approximately 04:30 on 7/21/05. Was given Motrin by parent. Shortness of breath started and went to ER. ER gave steroids, Benadryl and IV per mother.

Write-up: Induration surrounding injection site. 16 X 18cm area of edema surrounding site. Information from Follow-up report states I have received follow-up information on the VAERS report #241798. I spoke with the MD on this patient. She indicated the reaction to the Menactra vaccination. The patient tool Tylenol for 2 days, the swelling on the arm lasted for 5 days, and that the subject had guarded use of that arm. She state that he really didn''t even want to lift it, but would if he had to. No other symptoms were reported.

Write-up: Approximately 6-8 hours, post Menactra, experienced myalgias and fatigue and fever of maximum 101.5F. Symptoms all resolved with Tylenol and did not return.

Write-up: Received 2nd Hep A and a Menactra immunization on the morning of 7/27/05. Mother states about 4AM on 7/28 had vomiting without nauseam, sore throat, fever of 101, pale, dizzy, headache, and "fast heart beat". Symptoms relieved with Tylenol. Morning of 7/29 feeling much better and went to work. Discussed with mother that symptoms could be related to something other than the vaccine but that we would report her concerns.

Write-up: Phone call from pt''s mother 7/15/05 stating pt, her daughter, had Menactra 7/11/05 and had seizure that PM. Took her daughter to MD and referred to neurologist. States all test were negative.
