Write-up: Fever of 103F; whole body ache with stiff legs; severe headache. Fever decreased with Advil and Extra Strength Tylenol. Fever reoccurred a few hours later. Tx''d with Advil also headache.

Write-up: Around 2-3 hours after vaccines were given, pt developed sudden onset of leg pain and headache while sitting down. The pt was given ibuprofen. The pain progressed and she developed arm pain. She had difficulty walking secondary to the pain and a feeling of numbness in her legs. ("walked like a duck" per mom). She had no fever, no rash, no difficulty breathing. She was evaluated in the ER. After another 2 hours, pain was slightly improved where she could walk with less pain. Evaluated again ~24hours after vaccine in my office. Still c/o of pain, slight decreased strength in lower extremities, normal gait, no arm pain, no headache.

Write-up: 07/15/2005 - Right deltoid 11.7 x 15cm erythema, right axilla and chest 15 x 15cm erythema and edema tender to palpation. Treated with aspirin, cool compress. 07/18/2005 - resolved.

Write-up: Pt''s an 18 year old, status post meningitis shot, 2 days ago, had pain L arm. Also had generalized body aches. Arm site got swollen. Had fever yesterday of 101F with chills. No pain before shot. Today has pain underneath arm to shoulder joint. On exam, eyes slightly swollen. Dx''d with reaction to Menactra. Tx: Motrin Q6 hours and follow up is new symptoms.
