Write-up: Initial report received from a health care professional on 7/14/05. An 18 year old male patient received an IM left deltoid injection of Menactra, lot number U1494AB, and an ID right forearm injection of Tubersol 5 TU., lot number C2116AA, on 6/10/05. 15 days later he experienced intermittent tingling in his feet. He was seen by a physician on 6/26/05 and was admitted to the hospital to rule out Guillain-Barre Syndrome. He was released from the hospital on 6/29/05, but was re-admitted on 7/1/05 with facial weakness and slurred speech. He was diagnosed with Guillain-Barre Syndrome. The patient received treatment with plasmaphoresis and was discharged on 7/8/05. As per the reporter, the patient''s recovery status is unknown.

Write-up: Within 12 hours of receiving Menactra, pt developed headache, neck pain, spine pain, chills, subjective fever, difficulty standing and walking secondary to pain. Recommended fluids and Ibuprofen. Observed at office visit. Symptoms resolved in 24-48 hours.

Write-up: Received Menactra IM right arm approximately 1:00 PM on 7/18/2005. Woke at 4:00 AM 7/19/05 with fever chills, headache and slight nausea. Fever relieved by Tylenol but returned as it wore off. Slight redness @ site. Received Td at left deltoid also, at same visit. Upon return call to client on 7/20/05, signs and symptoms resolved.

Write-up: Patient received Menactra and PPD on 7/19/05. Patient reports itching began evening of 7/19/05, around waist line with progression to rash and hives on neck, stomach and groin the following morning. No respiratory difficulties reported. Hives and itching increased in severity during the day. She saw the nurse practitioner at approximately 3 p.m. and received Decadron 8 mg IM, a Medrol dose pack and Zyrtec p.r.n. The client''s mother reports this morning that the itching and hives are improving but not totally resolved. Patient also received tuberculosis skin test at the same time.

Write-up: One week after immunizations patient had a brief (3 minutes) generalized tonic-clonic seizure. Had low grade fever for several days prior to that. History of one previous seizure last year.
