Write-up: Pt had H/A, general malaise, myalgias, and generalized left arm pain. Pt felt "flu" like. No fever, no local reaction, on nuchal rigidity. Symptoms started about 5 hours after immunization, around 9:30 PM.

Write-up: On 6/9/2005 this patient received the Menactra Vaccine. Her mother called our office on 6/17/2005 stating she wasn''t sure if it was a reaction but wanted to let me know. On 6/15/2005 she developed generalized myalgia and arthrolgia with definite skin sensitivity telling her mom " don''t touch me, my skin even hurts". She had a temp of 101 with other sypmtoms of headache, sore throat, fatigue and general malaise. Symptoms lasted a couple of days. Mother is an RN and wanted it noted.

Write-up: Pt developed a localized erythematous, pruritic reaction at site of vaccine administration on left upper extremity. Pt noted itching and mild redness on day after administration. Area increased in size and became edematous and she noted tenderness and "bumps" in left axilla that resolved overnight. 2 days after vaccine administration she presented to the clinic for evaluation of reaction. At that time, a 5cm by 5cm area of erythema and edema was present on left upper arm. Area was soft, nontender, pruritic, without induration. There were no other signs or symptoms of allergic reaction.

Write-up: Sore arm, headache, stiff neck and nausea began on evening after mcv4 was administered. Symptoms resolving on 7/14/05. Heating pad to affected arm and tylenol given for pain with relief.

Write-up: Twenty four hours after receiving vaccination the patient began to develop white bumps around the vaccination site--went to her Dr. The bumps began spreading to her shoulder and, at time of interview, have spread to her back and antierior thorax--Dr ordered an antihistamine and a 7 day steroid dose pack.
