Write-up: This case was reported by a consumer and described the occurrence of thinking reduced in a female subject aged between 10 and 19 years old who was vaccinated with CERVARIX (GlaxoSmithKline). A physician or other health care professional has not verified this report. Historic vaccination included 1sst and 2nd doses of GARDASIL (GlaxoSmithKline, intramuscular, unknown site of injection and batch number) given on January 2011 and February 2011, respectively. In July 2011, the subject received 3rd dose of CERVARIX (intramuscular, unknown site of injection and batch number). In October 2011, 3 months after vaccination with CERVARIX, the subject''s academic results dropped (thinking reduced). The subject collapsed at home and experienced loss of consciousness. The subject also experienced depression, dyscalculia, consciousness disturbed, school refusal, mental concentration decreased and photophobia. As of 11 September 2014, the subject had been absent from school due to depression and underwent brain examination. At the time of reporting, the outcome of the events was unspecified. No further information was available, therefore the case was closed.

Write-up: Information has been received from SPMSD (manufacturer control # NO-1577272925-E2014-09060) on 03-OCT-2014. Case was received from a consumer or other non health professional via the Health Authority on 29-Sep-2014 under the reference NO-NOMAADVRE-PASRAPP-2014-0001106. Case is not medically confirmed. A 12-year-old female patient (weight 55 kg, height 164 cm) with no medical history reported received a dose of GARDASIL (batch no. NM18120, lot# NK44350, expire date 31-MAY-2012) via not reported route of site of administration on 15-Sep-2011. On 19-Dec-2011, the patient developed nausea, fever, abdominal pain and myalgic encephalomyelitis. On an unspecified date, the patient also developed flu-like symptoms. It started with cardiac tingling and cardiac pain, abdominal pain and nausea. The patient also experienced dizziness, vomiting, always palpitation, headache, influenza-like symptoms and pain in joints and muscles. Fever occurred on and off. In addition impaired muscles, shivering hands, feet gel-like on the inside of hands were reported. Further events included sensitive to light and sound, sleeps between 17-20 hours/day, concentration impairments, difficulties to think and loss of words. The patient had not the energy to go out, has not been to school since Sep-2013. According to the patient''s parents, she has developed myalgic encephalomyelitis due to this vaccine her entire adolescent and education/higher education have been shattered from this vaccine. Description of the adverse advent process: In Primary School they have documentation on how her absence increased post vaccination. HPV vaccine is not categorized under drugs, which have made the parents write something off topic to get their point through they feel that time has been lost, and it took too much time for the physician to deal with the case and to report it. The parents were desperate, since everything have been taken so much time, it took years. The parents reported the HPV vaccine. At the time of reporting, the patient had not recovered. On 14-Jun-2014, the patient received a further dose of GARDASIL. No further details reported. Noteworthy: Upon medical review the company judged relevant to code the following adverse events: cardiac pain, dizziness, vomiting, palpitations, which were not coded by the HA, but mentioned in the narrative.

Diagnostic Lab Data: She underwent brain magnetic resonance image (MRI) at a local hospital, which showed no abnormalities. She underwent an endoscopic examination and was given a diagnosis of superficial gastritis. She subsequently underwent detailed gynecological examinations due to the exacerbation of symptoms in the pre-menstrual period, the results of which showed no abnormalities. On an physical examination performed at our hospital, the patient was 146cm tall and weighed 45kg. Her pulse rate was 72 beats per minute (bpm), with a blood pressure (BP) of 98/50mmHg in the sitting position. The findings of general physical and neurological examinations were normal, except for slight tenderness in the left occipital area. Her skin temperature was 23.0 degrees Celsius in the right first toe and 28.5 degrees Celsius in the right second finger at a room temperature of 23.5 degrees Celsius, and plethysmograms of both toes and fingers disclosed a reduced height in the waves in the toes. The changes in the patient''s BP and plasma, level of noradrenalin on the Schellong test are presented. She did not exhibit orthostatic hypotension (OH), although the basal plasma, level of noradrenalin was low and the increase in the plasma, noradrenaline level was poor after changing positions from a supine to sitting position. These symptoms were consistent with the diagnostic criteria for orthostatic dysregulation (OD) proposed by the agency, including two major symptoms and four minor symptoms.

Write-up: This case was reported in a literature article and described the occurrence of headache in a 18-year-old female patient who received CERVARIX. In August 2011, the patient received CERVARIX (intramuscular). In October 2011, the dose was an unknown dose. In February 2012, the dose was an unknown dose. In August 2011, an unknown time after receiving CERVARIX, the patient experienced headache (serious criteria hospitalization and disability) and abdominal pain upper. On an unknown date, the patient experienced fatigue (serious criteria hospitalization and disability), mental disorder, insomnia, adjustment disorder, gastritis, somnolence, syncope, tenderness and orthostatic intolerance. On an unknown date, the outcome of the headache, fatigue, abdominal pain upper, mental disorder, insomnia, adjustment disorder, gastritis, somnolence, syncope, tenderness and orthostatic intolerance were recovering/resolving. It was not reported if the reporter considered the headache, fatigue, abdominal pain upper, mental disorder, insomnia, adjustment disorder, gastritis, somnolence, syncope, tenderness and orthostatic intolerance to be related to CERVARIX. In August 2013, an 18-years-old woman visited our hospital with complaints of continuous headaches and severe general fatigue. She had received her first dose of CERVARIX at the first week of August, 2011, and a few days later, began to suffer from severe epigastric pain and headaches. She underwent brain magnetic resonance image (MRI) at a local hospital, which showed no abnormalities; however, her headache persisted, accompanied by psychological instability and insomnia. Since she was unable to go to school, she was referred to a mental clinic and diagnosed as being inadaptable. During the first week in October, she received the second dose of the vaccine and her epigastric pain and headache worsened; her headache and general fatigue were more remarkable in the morning, and the administration of several types of drugs, including non-steroidal antiinflammatory drugs (NSAIDs), failed to relieve these symptoms. The patient underwent an endoscopic examination and was given a diagnosis of superficial gastritis, possibly due to psychological stress. In mid-February 2012, she received the third dose of the vaccine. At that time, her daily activity was severely disturbed by a difficulty in getting up, continuous heavy headaches and general fatigue. Finally, she stopped going to her regular high school and enrolled in a correspondence course at the beginning of May 2012. She was also treated by a psychiatrist; however, her symptoms did not improve. She subsequently underwent detailed gynecological examinations due to the exacerbation of symptoms in the pre-menstrual period, the results of which showed no abnormalities. A few months prior to admission, she began to experience frequent episodes of orthostatic fainting. On an physical examination performed at our hospital, the patient was 146cm tall and weighed 45kg. Her pulse rate was 72 beats per minute (bpm), with a blood pressure (BP) of 98/50mmHg in the sitting position. The findings of general physical and neurological examinations were normal, except for slight tenderness in the left occipital area. Diagnostic Lab Data: An MRI scan of the brain and routine laboratory tests were all within the normal limits. Her pulse rate was 80 bpm, with a BP of 90/55 mmHg in the sitting position. Her body temperature was 37.1C. In general, physical findings of coldness in both legs were noted, although the neurological gross power of the limb muscles was normal. Her skin temperature was 22.2C in the right first toe and 28.4C in the right second finger at a room temperature of 23.5C, and plethysmograms of the toe and finger showed markedly reduced heights of the waves. In contrast, the findings of peripheral nerve conduction studies were normal. Surface electromyograms recorded from the involved limbs indicated that the involuntary movements were not tremors, but rather myoclonus. On the Schellong test, her heart rate and BP changed from 78 bpm and 110/58 mmHg to 102 bpm and 67/48 mmHg, respectively, at eight minutes after standing. She was therefore diagnosed with OH.

In February 2011, the patient received CERVARIX (intramuscular). In September 2011, the dose was an unknown dose. In June 2012, an unknown time after receiving CERVARIX, the patient experienced eye pain and diplopia. In July 2012, the patient experienced muscular weakness (serious criteria hospitalization) and fall (serious criteria hospitalization). In July 2013, the patient experienced decreased appetite (serious criteria hospitalization), eating disorder (serious criteria hospitalization) and limb discomfort. In November 2013, the patient experienced headache, pain in extremity and gait disturbance. On an unknown date, the patient experienced complex regional pain syndrome, mental disorder, pain, clumsiness, panic reaction, hyperacusis, hyperaesthesia, dyskinesia, peripheral coldness, tremor, gait disturbance, myoclonus and motor dysfunction. On an unknown date, the patient experienced anxiety disorder and orthostatic hypotension. On an unknown date, the outcome of the muscular weakness and fall were recovered/resolved and the outcome of the decreased appetite, eating disorder, complex regional pain syndrome, mental disorder, headache, pain in extremity, eye pain, diplopia, pain, clumsiness, panic reaction, hyperacusis, hyperaesthesia, dyskinesia, limb discomfort, peripheral coldness, tremor, myoclonus and motor dysfunction were unknown and the outcome of the gait disturbance was recovering/resolving and the outcome of the anxiety disorder and orthostatic hypotension were unknown. It was not reported if the reporter considered the muscular weakness, fall, decreased appetite, eating disorder, complex regional pain syndrome, mental disorder, headache, pain in extremity, gait disturbance, eye pain, diplopia, pain, clumsiness, panic reaction, hyperacusis, hyperaesthesia, dyskinesia, anxiety disorder, limb discomfort, peripheral coldness, tremor, gait disturbance, myoclonus, motor dysfunction and orthostatic hypotension to be related to CERVARIX. In November 2013, a 15-year-old girl was referred to our hospital due to continuous headaches, limb pain and a gait disturbance. She had received her first dose CERVARIX at the end of February, 2011 and the third dose at the end of September, 2011. Her daily life had remained unchanged for a while; however, at the beginning of June, 2012 she began to complain of sharp pain in the eye balls and double vision, and, in the early morning in mid-July, she fell down after waking up due to left-sided limb weakness. She was transferred to an emergency hospital, where her consciousness was found to be clear, and an MRI scan of the brain and routine laboratory tests were all within the normal limits. The left-sided limb weakness recovered, and she was discharged. After this event, she frequently began to skip school due to an uncertain inching pain and clumsy movement in hands, which prevented her from writing or using chopsticks. Additionally, she became very sensitive to sound and skin stimuli, such as touch and using the shower in the bathroom, both of which easily induced a panic state, with involuntary tremulousness of the limbs, and she thus visited several hospitals, where she was given a diagnosis of a psychiatric or anxiety disorder. In early July, 2013, she developed severe anorexia, possibly due to very uncomfortable sensations in the limbs, and was hospitalized for two weeks. Although she was treated for an eating disorder, her condition did not improve, and after discharge, she remained at home. On a physical examination performed at our hospital, the patient was 150 cm tall and weighed 50 kg. Her pulse rate was 80 bpm, with a BP of 90/55 mmHg in the sitting position. Her body temperature was 37.1C. In general, physical findings of coldness in both legs were noted, although the neurological gross power of the limb muscles was normal. However, violent limb tremors were easily elicited whenever an examiner touched the patient''s thigh or legs. Due to this severe tremors, the patient was unable to walk alone. Her skin temperature was 22.2C in the right first toe and 28.4C in the right second finger at a room temperature of 23.5C, and plethysmograms of the toe and finger showed markedly reduced heights of the waves. In contrast, the findings of peripheral nerve conduction studies were normal. Surface electromyograms recorded from the involved limbs indicated that the involuntary movements were not tremors, but rather myoclonus. The patient''s reports of persistent inching pain, sensory changes with hyperesthesia and motor dysfunction with limb weakness and tremors, as well as our observation of these findings, were consistent with a diagnosis of CRPS-I, according to the guidelines. On the Schellong test, her heart rate and BP changed from 70 bpm and 110/58 mmHg to 102 bpm and 67/48 mmHg, respectively, at eight minutes after standing. She was therefore diagnosed with OH. The intradermal nerve pathology observed in the first toe and second finger is described in a separate section. The intravenous administration of physiological saline (100mL) and alprostadil; 5 mcg was employed to treat the leg coldness. The patient subsequently felt warmness in her limbs and the results of digital plethysmograms exhibited a normal pattern. After discharge, she continued to receive this treatment at her neighboring hospital, and the limb tremors gradually subsided. In addition, the patient again became able to walk and write and returned to school three month later.

On an unknown date, the subject received the 1st dose of CERVARIX (intramuscular). On 16th September 2011, the 2nd dose was an unknown dose. On an unknown date, the 3rd dose was an unknown dose. In January 2012, unknown after receiving CERVARIX, the subject experienced mental impairment (serious criteria GSK medically significant and clinically significant/intervention required). On an unknown date, the subject experienced muscular weakness, muscular weakness, dyskinesia, pain and asthenia. On 22nd September 2014, the outcome of the mental impairment was not recovered/not resolved. On an unknown date, the outcome of the muscular weakness, muscular weakness, dyskinesia, pain and asthenia were unknown. It was unknown if the reporter considered the mental impairment to be related to CERVARIX. The subject presented with various symptoms after receiving the second vaccination. Write-up: Information has been received from SPMSD, E2014-11309, on 24-NOV-2014). Case received from Patient Insurance Company on 19-Nov-2014 under the reference number 2014/04929. The primary reporter was a lawyer. The initial claim came from the patient''s mother. A 12 year old female patient with no medical history, had received second and third dose of GARDASIL (batch number not reported) via unspecified route of administration in unspecified site of administration on 28-Nov-2012 and 30-Apr-2013. After the second dose of GARDASIL she developed severe headache, muscle pain, muscle stiffness, muscle spasm, joint pain, joint stiffness, joint spasm, tiredness, low endurance for daily activity, difficulty walking and talking, cognitive problems, seizures, fuzzy head, nausea, vomiting, stomach pain, diarrhoea, constipation, flatulence, insomnia, loss of vision and visual disturbance, dizziness, shortness of breath, depression, anxiety, restless, decreased appetite, irregular menstruation, social phobia, other phobia, mood swings, ADHD, aggression, mental development disability, forgetfulness, personality changes and reduced quality of life. She developed events after her second dose of GARDASIL on 28-Nov-2012. The patient deteriorated every day since she was vaccinated. Her forgetfulness started and she complained that she forgot all she had learned or heard from her teachers, she got a bad vision and got spectacles in December 2012. She forgot things fast such as homework. She complained of severe headache, muscle and joint symptoms: pain, stiffness and spasm. Tiredness, low endurance for normal daily activity. Stroke-like symptoms: difficulty walking and talking, cognitive problems. Neurologic symptoms: seizures, fuzzy head, forgetfulness. Gastrointestinal symptoms: nausea, vomiting, stomach pain, diarrhea, constipation. Psychiatric symptoms: restlessness, anxiety and personality disorder. According to the patients mother the following symptoms started already in December 2012: neurological symptoms such as: severe headache, dizziness, visual disturbance and decreased appetite. She started to be unfocused in school and had problems being around noisy areas which were crowded with people. She could not keep up in school. She isolated herself from her friends, family and from social activities. She developed ADHD and mental development disability and aggression. She had often mood swings. She went from a smiling, happy and social girl to a depressed, unmotivated, anxiously child. She could not study in large classes, was lost in thoughts and in social development she had lost her healthy lifestyle. She was clever in school and at home, like to go swimming, bicycling, dancing and so on. After the vaccinations with GARDASIL she stopped being active, the life altered to the worst. Dreams of her carrier as psychologist was crushed when she could not keep up in school. She went from one of the best children to one of the worst in 2-3 months of time. The school reported to child welfare (10-Mar-2013) that the child was not following the program, isolated herself from school friends, cried often in class. Her mother contacted a physician and asked for an investigation in the end of 2012. On 10-Apr-2013 she was examined by child care family and PPT psychologist and was investigated for her school difficulties. She had several radiological examinations on the hospital and several blood samples. No decision was provided the company from Agency. At the time of reporting, the patient was not recovered. Upon medical review the company considered the case as serious.

A 28-year-old female patient with no medical history reported, had received three doses of GARDASIL (batch numbers not reported) via unspecified route of administration in unspecified site of administration on 17-Feb-2011, 21-Mars-2011 and 27-Jul-2011. The patient describes that the vaccines has given her new problems and reinforced old ones. She has taken various medications to treat the adverse reactions, it has however not improved her health or mental state. She describes that she is completely destroyed in mind and body and cannot function anymore. The patient has called in sick from her education on 31-Aug-2012. The patient''s description of her adverse reactions: Highly Sensitive Person HSP, but has now become over sensitive with everything. Her creative / imaginative abilities disappeared. She used to write poetry, write witty and eventful birthday greetings, writing stories / short stories, comment on other people''s postings on the social media etc. She cannot do all these things anymore. She went in early menopause and was bothered by all the symptoms at 39 years of age. Received medicine (not further specified). In Jan-2012, she lost her sex drive. She has tried to have sex one time in 2012 a week before she was 40 years old, but could not and has not desired sex either as she used to before. Irregular or absent menstruation. Taken CILEST (mfr other) since she was 17 years, she stopped taking them on 07-Sep-2012. Hormone cyst on the left ovary. Sponge and dry mucous membranes in the abdomen. Old injury on the left side of the skull (depressions in the skin after a hard blow from a swing''s metal bracket) it has become more evident now. She thought she had processes old trauma, but it showed up with all the old emotions and pain. She has new trauma due to her illness. Loss of appetite, anorexia, altered taste buds and weight loss. Balance, motor / coordination and sense of direction disorders. Her balance is very poor in the left side of the body. Clumsy, trips, goes into things, loses things, had difficulty recognizing places, directions from right and left. Inability to judge distances. Muscles, tendons, joints and movement pain. Neck, shoulders, spine, chest, arms, wrists, fingers, lower back / hip, the left buttock. Groin pain in the left leg and pain in the left foot / toe. Write-up: This case was reported by a physician and described the occurrence of cerebration impaired in a 17-year-old female subject who received CERVARIX (batch number AHPVA100BC, expiry date unknown) and (batch number AHPVA122AA, expiry date unknown. On 5th February 2011, the subject received the 1st dose of CERVARIX (intramuscular). On 5th March 2011, the 2nd dose was an unknown dose. On 5th March 2011, 28 days after receiving CERVARIX, the subject experienced syncope (serious criteria clinically significant/intervention required). In 2014, the subject experienced cerebration impaired (serious criteria GSK medically significant and clinically significant/intervention required). On an unknown date, the subject experienced weakness (serious criteria clinically significant/intervention required), dystonic reaction (serious criteria clinically significant/intervention required), gait disturbance (serious criteria clinically significant/intervention required) and motor dysfunction (serious criteria clinically significant/intervention required). The subject was treated with. On an unknown date, the outcome of the cerebration impaired, syncope, weakness, dystonic reaction, gait disturbance and motor dysfunction were not recovered/not resolved. It was unknown if the reporter considered the cerebration impaired, syncope, weakness, dystonic reaction, gait disturbance and motor dysfunction to be related to CERVARIX. After the second vaccination, the subject experienced syncope at home. Thereafter, various symptoms developed including repeated episodes of syncope, weakness (asthenia), dystonic symptom (dystonia), gait disturbance, higher motor dysfunction, and higher brain dysfunction (mental impairment). In March 2014, the subject visited Hospital and received steroid pulse therapy.

An 11 year old female patient with a medical history of allergy rhinitis, URTI, allergy to animals and grass allergy, had received dose 1 of GARDASIL (batch and lot number J004774, exp. 31-AUG-2015) via i.m. route of administration in unspecified site of administration on 23-Oct-2014 and later on the same day she developed malaise, tiredness and dizziness. On 25-Oct-2014 she developed vomiting, on 02-Nov-2014 pain in back head and headache and on 03-Nov-2014 she developed difficulties walking without support. A report from a school physician about an 11 year old girl who was vaccinated with GARDASIL dose 1. The girl had a little runny nose pre-vaccination and felt unwell after vaccination, tired and dull. Vomited in the evening 2 days after vaccination, tired, 9 days after vaccination the girl developed a headache and "spaghetti legs", another day later; pain in the back of the head and difficulties walking without support. The patient was examined at the hospital, neurological status and tests showed normal except for a thyroid test which showed slightly abnormal (not further specified). The girl was still very tired with some nausea. Investigated at the hospital without any remarks. The outcome is unknown but the girl is back at school. Adverse events are in the report classified as serious, due to hospitalization, by the reporter. At the time of reporting, the outcome was unknown for all the adverse events, except for vomiting which was recovered. According to the HA, the reactions were possibly related to vaccination.

Write-up: Initial information has been received from a physician via the Agency concerning a 12 year old girl who on 20-AUG-2013 was placed on therapy with GARDASIL (intramuscular) (dose not reported, lot no. 0989AA/9QN08R, expiry date 21-JUN-2014). No information regarding special notes on the pre-vaccination interview form (including underlying disease, allergy, vaccination/disease within the last 1 month, medications currently taken, history of adverse drug reaction and growth status) or family history was obtained. Other concomitant medications were not reported. On 25-MAY-2013, the patient received GARDASIL (intramuscular) for the first time (lot no. 0989AA/9QN08R, expiry date 21-JUN-2014). On 20-AUG-2013, the patient received GARDASIL (intramuscular) for the second time (lot no. 0989AA/9QN08R, expiry date 21-JUN-2014). In around the end of August 2013, the patient experienced cramps in the calves, pain legs and photosensitivity after the second vaccination. Subsequently the patient spread to the entire body, and the typical symptoms included disturbance of thermal sensation and symptoms of higher brain function deterioration were observed (the patient developed vasovagal reaction, cramps in the calves, pain legs, photosensitivity, pain spread to the entire body, disturbance of thermal sensation and symptoms of higher brain function deterioration). At the time of the report on 14-JAN-2015, the patient had not recovered from vasovagal reaction, cramps in the calves, pain legs, photosensitivity, pain spread to the entire body, disturbance of thermal sensation and symptoms of higher brain function deterioration. Reporter''s comment: The symptoms became severe. The reporting physician considered that vasovagal reaction, cramps in the calves, pain legs, photosensitivity, pain spread to the entire body, disturbance of thermal sensation and symptoms of higher brain function deterioration were serious due to disability. The reporting physician felt that vasovagal reaction, cramps in the calves, pain legs, photosensitivity, pain spread to the entire body, disturbance of thermal sensation and symptoms of higher brain function deterioration were related to GARDASIL (intramuscular). Additional information has been requested.
