Write-up: Pt was given Hep-A vaccine at 18:00-18:15. He was a chaplain with the Police Dept and was riding with a Police Officer. The Police Officer took the pt home at 23:30. States that the pt had no complaints at that time. At 02:15, the Police Dept was informed by the pt''s wife that he had been sent to the hospital and died. Autopsy states cause of death as atherosclerotic cardiovascular disease. Other significant conditions: diabetes, hypertension, obesity.

Write-up: This report is concerning a 29 year old female who on 2/13/01 was vaccinated with Hep-A and Japanese encephalitis vaccines. On 2/13/01, at 19:40, the pt complained of feeling hot, sweaty and slightly faint. The pt was placed in a supine position; BP was 134/70 and pulse was 94. The physician was notified immediately. At 19:41 the pt''s BP was 131/70 and pulse was 70. The pt complained of tightness in the chest, very anxious and a funny feeling in he chest. Epinephrine was given IM at 19:45 and at 19:53 the pt stated she was not feeling good, Bp was 155/67 and pulse was 111. At 19:57, her BP was 163/75 and pulse was 101. The pt was still shaking. The pt had the following BP and pulse measurements: 163/75, pulse 117; 136/68 pulse 101; 149/67, pulse 113, 142/57. At 20:20, Benadryl was given IM with an immediate BP of 120/67 and pulse of 141. At 20:23, oxygen given 2L via nasal cannula. The pt''s BP was 154/57 and pulse 127, 145/60 and pulse 119, 154/62 and pulse 129, 155/64 and pulse 131. The pt was transferred to the hospital. The pt reportedly recovered from the experience. This was originally reported by a health care professional. Additional information has been requested.

Write-up: Pt developed leg weakness and atrophy which was slowly progressive (left leg worse than right leg). This was progressive over 6 months to the point that she could no longer walk unassisted. She was admitted and dx''d with anterior horn cell lesion and received inpatient rehabilitation.

Write-up: About less than 10 minutes, post vax, the pt said he was not feeling good. He was brought back to the examining room and found to be pale and weak. He was put on the supine position, but did not respond well and became bradycardic and very pale; responded briefly to smelling salts and better after an injection of Epinephrine. He remained hypotensive BP 70/40; IV bolus was give an sent to ER by EMS.

Write-up: Complained of petechial bruises. Saw doctor on 8/9/00, 2 days, post vax, with bruises on legs, sores in mouth, blood on lips in the AM for 2 days. No recent illness or tick or insect bites. Swelling 1+ at inguinal nodes and 2+ at axillary nodes; bruises and petechia. Treated with Prelone. On 8/22/00, phone call from mom. She wants to stop sibling from receiving IZ''s based on what happened with this son and advised her to speak with the doctor about the advantages and disadvantages and she said she would consider this and call back.

Write-up: Fever of 105F; decreased slightly with Tylenol (given X1). By 08:00 AM, father concerned that son had a seizure. MD visit and admitted to the hospital for observation. Pt had slight diarrhea. Parent states client had a headache the morning of 4/4/01 but was well at the time of shots. No seizure noted in the hospital. Parent states client was aphasic for a short time. Discharged without dx of symptoms and clear tests.

Write-up: This patient complained about arms hurting and getting hot. He then had multiple episodes of vomiting. The patient got very pale with staring, and his heart rate increased. He then became limp and lifeless and eyes rolled back. This lasted for about two minutes. While hurrying to the ER, the patient went into a seizure and the mother even thought he had died. The ER doctor said it was probably a reaction the the shots.

Write-up: Information has been received from a physician concerning an adult male who was vaccinated with one dose of Hepatitis A virus vaccine inactivated. Thirteen days post-vaccination the patient developed a headache so severe he had to be hospitalized. An MRI was performed to r/o an aneurysm, however, at the time of reporting no results were available. The headache "has lasted for about one week now". Additional information has been requested.
